Karbon Moniksit Zehirlenmesinde Serebrovasküler Değişiklikler ve EEG Bulguları: Olgu Sunumu

Abstract

Karbonmonoksit (CO) zehirlenmesi, zehirlenmeye bağlı ölümlerin en
sık nedenlerindendir. CO’in, hemoglobine (Hgb) bağlanarak oluşan
karboksihemoglobinin dokularda hipoksiye yol açtığı bilinmektedir.
CO zehirlenmesinde geç nörolojik bozukluklara ileri yaşlarda daha sık
rastlanıldığı bildirilmiştir. CO zehirlenmesinde akut dönemde serebral iskemi
bulguları ve beyindeki işlev bozukluğuna bağlı EEG’de yaygın yavaş dalga
aktivitesi ile lateralizasyon gösteren diken dalgalar gösterilmiştir. Bu yazıda
CO zehirlenmesi tanısı ile acil servise müracaat eden ve yoğun bakımda
takibi yapılan 77 yaşındaki hastada ortaya çıkan nörolojik bulguların EEG
ve MR ile tespitini bildirmek istedik.

Cerebrovascular Changes and EEG Findings in Carbon Monoxide Intoxication: Case Report

Abstract

Carbon monoxide (CO) intoxication is one of the most seen causes of death
due to intoxication. CO has affinity to hemoglobin. Hemoglobin and CO
complex carboxyhemoglobin (COHgb) causes hypoxia in tissue. In acute
phase of CO intoxication generalized slow wave activity and lateralized pike
waves could be seen on EEG. Late onset neurological deficits due to CO
intoxication were seen more in elder patients. In this report we define a case
of 77 years old age woman who was managed in intensive care unit due to
CO intoxication with unconsciousness in which the neurological defects are
shown with MRI and EEG.
