METOKLOPRAMİD KULLANIMINA BAĞLI GELİŞEN AKUT DİSTONİ: İKİ OLGU SUNUMU

Abstract

Metoklopramid, dopamin reseptör antagonisti bir benzamid olup,
gastroözefagial reflü hastalığının tedavisinde gastrointestinal pasajı
hızlandırmak için prokinetik bir ajan olarak ve bulantı-kusma ile seyreden
birçok hastalıkta antiemetik olarak sıklıkla tercih edilir. Sindirim
sistemi üzerine hem santral hem de periferik etkilidir. Kan-beyin engelini
kolayca aşar ve ekstrapiramidal sisteme ait yan etkiler oluşturabilir.
Bu yan etkileri arasında akut distonik reaksiyon nadirdir ancak acil
tedavi gerektiren bir durumdur.Bu yazıda, ilki 29 yaşında erişkin ve
terapötik dozda metoklopramid kullanmasına rağmen, diğeri 9 aylık
bir bebeğe hekim önerisiyle yüksek dozda metoklopramid verilmesine
bağlı olarak akut distonik reaksiyon gelişen 2 ayrı olgu sunuldu. Sunulan
her iki olguda tanı hikaye ile konuldu ve tedaviye dramatik olarak
yanıt alındı. Akut distoni ile acil servise başvuran hastalarda, sık
kullanılan bir antiemetik olan metoklopramidin yan etkisinin olabileceği
unutulmamalı ve bu durumda, hastalarda mutlaka ilaç kullanım öyküsü
sorgulanmalıdır. Ayrıca, metoklopramide bağlı distonik reaksiyonların
doz-aşımına bağlı ya da dozdan bağımsız olarak idiosenkrazik olarak
ortaya çıkabileceği hatırlanmalı, bu yüzden doz ayarlaması esnasında
daha özenli davranılmalıdır

ACUTE DYSTONIA SECONDARY TO USE OF METOCLOPR- AMIDE: TWO CASE REPORTS

Abstract

Metoclopramide is a dopamine receptor antagonist benzamide derivative, and is frequently preferred as a prokinetic agent to accelerate gastrointestinal passage in the treatment of gastroesophageal reflux disease and antiemetically in many diseases presenting with nausea and vomiting. It has both central and peripheral effects on the digestion system. It can easily overcome the blood-brain barrier and may create side effects on the extrapyramidal system. Among these side effects, acute dystonic reaction is an uncommon yet a situation needing emergency treatment. In this study, we present 2 separate cases with acute dystonic reactions; the case of a 29-year-old adult with reactions despite using metoclopramide in therapeutic doses, and the case of a 9-month infant with reactions secondary to high doses of metoclopramide administration. Diagnosis was made via anamnesis in both of the presented cases, with the treatment eliciting a dramatic response. It should be kept in mind that metoclopramide, a commonly used antiemetic in patients referred to the emergency service with acute dystonia, might have side effects. In this setting, a history of medication use must be examined from the patients, absolutely. Additionally, ıt should be remembered that dystonic reactions secondary to metoclopramide might occur idiosyncratically depending on overdose or independent from dosage thus, more care needs to be taken during dose adjustment
