Abstract

Sünnet derisinin (prepisyum) veya glans penisin fermuara sıkışması
özellikle çocuklarda sık rastlanan bir ürogenital yaralanmadır. Bu durum,
genellikle sünnetsiz erkek çocuklarda karşımıza çıkar ve oldukça
ağrılı olabilir. Sünnet derisini fermuardan kurtarmak için birkaç yöntem
tarif edilmiştir. Bunların arasında fermuarı tel veya kemik kesici ile
sökmek, fermuar kilidinin yan taraflarına bir dişli pens yardımıyla bası
uygulamak yada lokal anestetik ve lubrikan (kayganlaştırıcı) özellikte
bir madde uygulandıktan sonra ani bir hareketle fermuarın sıkışmanın
olduğu aksi yöne çekilmesi sayılabilir. Bu yazıda sünnet derisinin
fermuara sıkışması şikâyetiyle acil servise başvuran 6 yaşındaki bir
çocuğun yönetimindeki ağrısız ve güvenli yaklaşım esasları sunuldu.

Abstract

Zipper entrapment of the prepuce or the glans penis is a urogenital injury occurring frequently particularly in children. This is generally encountered in the case with uncircumcised male children and may be a very painful process. Several methods are described to release the prepuce from the zipper. Among them are removing the zipper by the help of a wire or a bone cutter, applying pressure on the sides of the zipper lock with a toothed forceps or suddenly drawing the zipper contrary to the entrapment direction after the application of local anesthetic agents and lubricants. This study presents the principles of a painless and safe approach in the management of 6-year old child referred to the emergency service for zipper entrapment of the prepuce
