Is lower limb-based task-oriented training effective on balance parameters in stroke patients?

Abstract

Purpose: The aim of this study is to compare the effects of conventional physiotherapy and rehabilitation currently applied on stroke patients and lower limb-based task-oriented training on balance parameters. Methods: The study included a total of 39 stroke individuals who are 18 and over, had stroke for the first time, Brunnstorm lower limb stage≥3, functional ambulation classification≥2, had no additional neurological problems for at least 3 months after stroke. The cases were divided into two groups one of which is treated with conventional method (C-PTR) and the other with lower limb based structured task-oriented training (LBTO-PTR). All the individuals were given treatment for 4 weeks, 5 days a week, an hour a day. The patients were evaluated in terms of Berg balance scale (BBS), functional reach test (FRT), postural symmetry, weight transfer symmetry and stroke impact scale (SIC) at the beginning and at the end of the treatment. Results: C-PTR and LBTO-PTR groups were both evaluated before and after the treatment; a statistically significant increase was observed in BBS, FRT and SIC (p<0.05) while there was no difference in postural symmetry and weight transfer symmetry (p>0.05). No difference was observed between the groups when C-PTR and LBTO-PTR groups were compared after treatment (p>0.05). Conclusion: The positive effects of lower limb based structured task-oriented training on balance in stroke patients show similarities with conventional physiotherapy and rehabilitation training.

İnmeli hastalarda alt ekstremite temelli yapılandırılmış hedef odaklı eğitim denge parametreleri üzerinde etkili midir?

Abstract

Amaç: İnmeli hastalarda hali hazırda yaygın olarak uygulanan konvansiyonel fizyoterapi ve rehabilitasyon programıyla, alt ekstremite temelli yapılandırılmış hedef odaklı eğitimin denge parametreleri üzerine olan etkilerinin karşılaştırılmasıdır. Yöntem: Çalışmaya, 18 yaş ve üzeri, ilk defa inme geçirmiş, Brunnstorm alt ekstremite evre ≥3, fonksiyonel ambulasyon sınıflaması ≥2 olan, inme sonrası en az 3 ay geçmiş, herhangi ek nörolojik problemi olmayan 39 inmeli birey dahil edilmiştir. Olgular, konvansiyonel yöntemle tedavi edilen (K-FTR) ve alt ekstremite temelli yapılandırılmış hedef odaklı eğitimle tedavi edilen (ATHO-FTR) grup olmak üzere ikiye ayrılmıştır. Bütün bireyler 4 hafta boyunca haftada 5 gün birer saat boyunca tedaviye alınmıştır. Tedavinin başında ve sonunda Berg Denge Ölçeği (BDÖ), Fonksiyonel Uzanma Testi (FUT), postüral simetri, ağırlık aktarma simetrisi ve İnme Etki Skalası (İES) değerlendirmeleri yapılmıştır. Bulgular: K-FTR ve ATHO-FTR gruplarının her ikisinde tedavi öncesi ve sonrası değerlendirmelerde BDÖ, FUT ve İES’de istatistiksel olarak anlamlı artış gözlenirken (p<0,05), postüral simetri ve ağırlık aktarma simetrisinde fark gözlenmemiştir (p>0,05). K-FTR ve ATHO-FTR grupları tedavi sonrası birbirleri ile karşılaştırdıklarında ise gruplar arası fark bulunmamıştır (p>0,05). Sonuç: İnmeli hastalarda alt ekstremite temelli yapılandırılmış hedef odaklı eğitimin denge üzerindeki olumlu etkileri konvansiyonel fizyoterapi ve rehabilitasyon eğitimi ile benzerlik göstermektedir.
