Serebral palsili çocuklarda nörogelişimsel tedaviye dayalı gövde eğitiminin gövde kontrolüne etkisi

Abstract

Amaç: Bu çalışmada, spastik diplejik serebral palsili (SP) çocukların motor fonksiyonlarını yerine getirmede önemli yer tutan gövde kontrolü ve gövde kontrolüne yönelik egzersizlerin kas kuvveti ve denge üzerine olan etkisinin değerlendirilmesi amaçlandı.

Yöntem: Çalışmaya Kaba Motor Fonksiyon Sınıflama Sistemi’ne (KMFSS) göre seviyeleri I, II ve III olan, yaşları 3-10 yıl arasında değişen 40 spastik diplejik SP’li çocuk dahil edildi. Çocuklar egzersiz ve kontrol grubu olarak iki gruba ayrıldı. Egzersiz grubuna, haftada 2 gün 45 dakika süren 6 hafta boyunca konvansiyonel fizyoterapi programlarına ek olarak gövde kontrolüne yönelik Nörogelişimsel Tedavi - Bobath (NGT-Bobath) programı uygulandı. Kontrol grubu ise, haftada 2 gün 45 dakika süren konvansiyonel fizyoterapi programlarına devam etti. Kontrol grubunun mevcut tedavi programına herhangi bir müdahalede bulunulmadı. Gruplara başlangıçta ve 6 hafta sonra Pediatrik Berg Denge Ölçeği, Gövde Kontrol Ölçüm Skalası uygulandı. Çocukların gövde kas kuvvetleri ayrıca değerlendirildi.

Bulgular: Tedavi sonrasında egzersiz grubu ile kontrol grubu arasında Pediatrik Berg Denge Ölçeği sonuçlarında ve gövde kas kuvvetinde egzersiz grubu lehine fark bulundu (p<0.05). Ayrıca egzersiz grubunun tedavi sonrası; Gövde Kontrol Ölçüm Skalası, Pediatrik Berg Denge Ölçeği ve tüm gövde kaslarının kuvveti tedavi öncesine göre anlamlı iyileşme gösterdi (p<0.05).

Sonuç: Bu çalışma, SP’li çocuklardaki klasik fizyoterapi ve egzersiz programına gövdeye yönelik egzersizlerin eklenmesinin kas kuvvetini artırdığını ve dengeyi olumlu yönde etkilediğini gösterdi.
