Halluks valgusu olan kadınlarda egzersiz ile kombine gece ortezi ve bantlama yöntemlerinin etkilerinin karşılaştırılması: pilot çalışma

Abstract

Amaç: Bu çalışmanın amacı egzersiz ile kombine gece ortezi ve bantlama uygulamalarının halluks valgus (HV) deformitesi, ağrı
şiddeti ve yaşam kalitesi üzerine etkilerini karşılaştırmaktı.


Yöntem: Çalışmaya 10 kadın hastanın 17 (bilateral:7, unilateral:3) ayağı dahil edildi. Bireyler, ortez grubu ve bantlama grubu
olarak ikiye ayrıldı. Ortez grubuna HV gece ortezi, bantlama grubuna ise rijit bant 8 hafta süreyle uygulandı. Ayrıca bütün
katılımcılar tarafından günde bir kez, 15 tekrarla, toplam sekiz hafta süreyle HV egzersizleri uygulandı. Bireylerin halluks valgus
açıları (HVA) ve intermetatarsal açıları (IMA) radyografik yöntem ile başlangıçta ve tedaviden hemen sonra değerlendirildi.
Çalışmada kullanılan diğer değerlendirme yöntemleri ise Ayak Fonksiyon İndeksi (AFİ) ve Manchester-Oxford Ayak Anketi
(MOXFQ) idi. Gruplar arası fark Mann Whitney U testi ile analiz edildi. Anlamlılık düzeyi p<0,05 olarak belirlendi.

Bulgular: Grupların tedavi öncesi ile tedavi sonrası ortalama değerleri arasındaki farklar karşılaştırıldığında; HVA’nda bantlama
grubu lehine anlamlı fark olduğu gözlendi (p<0,05). MOXFQ yürüme ve ayakta durma alt skalası sonuçları ise ortez grubu lehine
anlamlı fark olduğu gözlendi (p<0,05).

Sonuç: Çalışmamızın sonuçları, bantlama uygulamasının deformitenin azaltılmasında daha etkili olduğunu, gece ortezinin ise
yürüme ve ayakta durma fonksiyonunu iyileştirmede daha etkili olduğunu gösterdi. Bu sonuçlar bize ortez ve bantlama
uygulamalarının HV tedavisinin farklı parametrelerinde birbirlerine göre üstün olduğunu göstermektedir. Klinikte hasta yararı
gözetilerek uygun tedavi yöntemi tercih edilmelidir.
