Çeşitli klinik örneklerden izole edilen stafilokok suşlarında slime oluşumu ve antibiyotik direnci arasındaki ilişkinin araştırılması

Abstract

Bu çalışmanın amacı Şubat-Nisan 2011 tarihleri arasında Özel Lokman Hekim ve Etlik İhtisas Eğitim ve Araştırma Hastaneleri Mikrobiyoloji Laboratuvarları’ndan alınan, çeşitli klinik örneklerden (yara, kan, trakeal aspirat, balgam, idrar, eklem sıvısı, kateter, kulak) izole edilen 100 adet stafilokok suşunun slime oluşturma özellikleri ve antibiyotik direnci arasındaki ilişkiyi araştırmaktır. Suşların slime oluşturma özellikleri Kongo Kırmızılı Agar (CRA) yöntemiyle, antibiyotik duyarlılıkları ise Clinical Laboratory Standards Institute (CLSI) önerileri doğrultusunda Kirby-Bauer disk difüzyon yöntemi ile saptanmıştır. Gram boyama, katalaz testi, koagülaz testi ve hemoliz oluşturma özelliklerine göre identifiye edilen suşlardan 42 tanesi Staphylococcus aureus, 58 tanesi koagülaz negatif stafilokok (KNS) suşu olarak tanımlanmıştır. 58 adet KNS suşunun 17 tanesi ve 42 adet S.aureus suşunun 20 tanesi slime pozitif olarak belirlenmiştir. Suşların tamamı vankomisine, teikoplanine ve linezolide duyarlı bulunmuştur. Oksasilin direnci slime pozitif suşlarda daha yüksek saptanmış olup, en yüksek slime pozitif KNS’larda saptanmıştır. Slime pozitif suşların tamamı, slime negatiflerin büyük çoğunluğu penisilin ve ampisiline karşı direnç göstermiştir. Siprofloksasin, ofloksasin, eritromisin, sefazolin, gentamisin, klindamisin, tetrasiklin ve rifampisin direnci slime pozitif suşlarda, negatif suşlara oranla daha yüksek bulunmuştur. Slime oluşturan suşların antibiyotiklere daha yüksek oranda direnç gösterdiği belirlenmiştir

Abstract

The purpose of this study was to investigate the relation between slime production and antibiotic resistance of 100 staphylococci isolated from various clinical specimens (wound, blood, tracheal aspirates, sputum, urine, synovial fluid, catheter tip, ear) at Etlik Lokman Hekim and Etlik Training and Research Hospitals, Ankara, Turkey between February-April 2011. Slime production was determined by Congo Red Agar (CRA) method andantibiotic susceptibilities were determined by Kirby-Bauer disk diffusion method according to the Clinical and Laboratory Standards Institute recommendations. Gram staining, catalase testing, coagulase testing and hemolytic properties were used to identify isolates. According to the results 42 isolates were determined as Staphylococcus aureus and 58 isolates were determined as coagulase negative staphylococci (CNS). Slime production was detected in 17 of 58 CNS and 20 of 42 S.aureus. All isolates tested were susceptible to vancomycine, teikoplanin and linezolid. Resistance to oxacillin in slime positive isolates were detected higher than the slime negative strains, the highest resistance was detected in CNS. All of slime positive strains and most of slime negatives were resistant to penicillin and ampicillin. Resistance to ciprofloxacin, ofloxacin, erythromycine, cefazolin, gentamicin, clindamycin, tetracycline ve rifampicin were detected higher in slime positive strains compared to negative strains. As a result, slime positive isolates perform higher resistance to antibiotics
