PERFORMANCE AND EVALUATION OF THE TURKISH HEALTH SYSTEM

Abstract

The structure, processes and outcomes of health systems basically aim to raise the health status of the society. The purpose of this study; To analyze and make recommendations based on comparison of the results of the Turkish Health System both internally and internationally. The study data belongs to 2021 and was obtained from the Ministry of Health Health Statistics Yearbook, TURKSTAT, OECD Health Data and Eurostat database. Within the scope of the literature in order to evaluate the results of the health system; Average life expectancy, infant mortality rate, maternal mortality rate, patient satisfaction and health expenditure impact data were taken into consideration. The data provided was analyzed by making comparisons between regions at the country level and also at the international level. Within the scope of the study findings, it has been observed that Turkey has achieved an increasing momentum in terms of life expectancy at birth, but has not achieved the desired performance at the international level in terms of Infant Mortality Rate and Maternal Mortality Rate, and there are extreme differences between regions. Although patient satisfaction generally appears to be at a good level internationally in terms of comparison, it has been determined that it has entered a period of stagnation and decline. It has been observed that health expenditures have a positive impact on the health system results, average life expectancy and patient satisfaction. As a result, it has been observed that the Turkish Health System is in the process of transition from middle-upper income group countries to upper income group countries in terms of its results and performance, but the imbalances in access to service and health service quality between regions produce negative results. In this context, it can be said that there is a need for policies for balanced distribution and access to maternal and infant health services throughout the country, as well as regulations for the adaptation of the private health sector to the society and reforms for the quality of health services in terms of sustainability of patient satisfaction.

TÜRK SAĞLIK SİSTEMİNİN PERFORMANSI VE DEĞERLENDİRİLMESİ

Abstract

Sağlık sistemlerinin yapısı, süreçleri ve çıktıları temelde toplumun sağlık statüsünü yükseltmeyi amaçlamaktadır. Bu çalışmanın amacı; Türkiye Sağlık Sistemi’nin sonuçlarının hem kendi içinde hem de uluslararası düzeyde karşılaştırmaya dayalı olarak analizini yapmak ve önerilerde bulunmaktır. Çalışma verileri 2021 yılına ait olup Sağlık Bakanlığı Sağlık İstatistikleri Yıllığı, TÜİK, OECD Sağlık Verileri ve Eurostat veri tabanından temin edilmiştir. Sağlık sistemi sonuçlarını değerlendirmek amacıyla literatür kapsamında; ortalama beklenen yaşam süresi, bebek ölüm hızı, anne ölüm hızı, hasta memnuniyeti ile sağlık harcamaları etkisi verileri dikkate alınmıştır. Temin edilen veriler ülke düzeyinde bölgeler arasında ve ayrıca uluslararası düzeyde kıyaslamalar yapılarak analiz edilmiştir. Çalışma bulguları kapsamında Türkiye’nin doğumda beklenen yaşam süresi açısından yükselen bir ivme yakaladığı ancak Bebek Ölüm Hızı ve Anne Ölüm Hızı açısından uluslararası düzeyde istenen performansı yakalayamadığı ve bölgeler arasında aşırı farklılıklar olduğu görülmüştür. Hasta memnuniyeti ise genel olarak kıyaslama açısından uluslararası olarak iyi düzeyde görünmekle birlikte durağanlık ve gerileme dönemine girdiği tespit edilmiştir. Sağlık harcamalarının sağlık sistem sonuçlarından ortalama yaşam süresi ve hasta memnuniyetine olumlu etki yaptığı görülmüştür. Sonuç olarak Türkiye Sağlık Sistemi sonuçları ve performansı itibarı ile orta-üst gelir grubu ülkelerden üst gelir grubu ülkelere geçiş sürecinde olduğu ancak bölgeler arasında yaşanan hizmete erişim ve sağlık hizmet kalitesi dengesizliklerinin olumsuz sonuçlar ürettiği görülmüştür. Bu kapsamda anne ve bebek sağlığı hizmetlerinin ülke çapında dengeli dağılımı ve erişimine yönelik politikaların yanı sıra özel sağlık sektörünün topluma adapte edilmesine yönelik düzenlemeler ile hasta memnuniyetinin sürdürülebilirliği açısından sağlık hizmet kalitesine yönelik reformlara ihtiyaç olduğu söylenebilir.
