SUMMARY
Although the selective grinding of cusped teeth in complete dentures is a very important step for a successful prognosis of the case, it is nevertheless often overlooked by the general practitioner.
Every prostheses has to be adjusted to eliminate the processing errors inherent in the properties of the materials, and also to correct minute discrepancies due to the resiliency of the soft tissues.
New centric registration is necessary and centric occlusion has to be corrected to coincide with centric relation. The supporting cusp tips, i.e. Buccal of the lowers and Lingual of the uppers, are not to be touched unless they interfere in centric and all eccentric movements as well. Thus the occlusal vertical dimension will be preserved.
Prolvusive is corrected next : i.e. disto-lingual inclines of the upper buccal cusps and/or mesio-buccal inclines of the lower lingual cusps. In the anterior area incisal edges of the lower anteriors are to be adjusted rather than the upper anterior teeth set for a pleasing esthetics.
Working areas of spot grinding are the mesio-lingual inclines of the upper buccal and/or the mesio-distal lingual inclines of the lower lingual cusps.
On the balancing side the adjustments would be on the disto-buccal inclines of the upper lingual cusps and/or the mesio-lingual inclines of the lower buccal cusps.
The polishing of artificial teeth should not be omitted.
