GÖMÜK 20 YAŞ DİŞLERİNİN ÇEKİM NEDENLERİ

Abstract

Gömük 20 yaş dişlerinin cerrahi çekimleri diş hekimliği kliniklerinde en çok yapılan uygulamalardan biridir. Gömük 20 yaş dişlerinin çekim endikasyonları açıkça belli olmasına rağmen asemptomatik 20 yaş dişlerinin profilaktik çekimleri de oldukça yaygındır ancak bu yaklaşım uzun zamandan beri tartışma konusudur. Klinik olarak gerek olmadığı halde profilaktik amaçlı çekilen gömük yirmi yaş dişlerinin oranı %18 ila %50 arasında değişmektedir. Profilaktik diş çekimini savunan hekimlerin gerekçeleri arasında ileride 20 yaş dişlerinin yol açabileceği rahatsızlıkların ve patolojilerin önüne geçilmesi, mandibular fraktür riskinin azaltılması, yaşla birlikte cerrahi müdahalenin zorlaşması ve yirmi yaş dişlerinin ağız içerisinde fonksiyona sahip olmamasıdır. Bu uygulamaya karşı çıkan hekimlerin görüşü ise hastaların gereksiz cerrahi tedavi sonucu travmaya maruz kalmaları, buna bağlı olarak ağrı duymaları ve ekonomik yük altında kalmalarıdır. Bu derlemenin amacı gömük dişlerin cerrahi çekimine karar vermeden önce hekimlerin göz önünde bulundurması gereken noktaları yeniden irdelemektir.

Abstract

Extraction of impacted third molars is one of the most performed surgical interventions in the oral surgery departments. Although the indications for removal of impacted third molars are well described, prophylactic removal of these teeth are still a common practice among the clinicians and a topic widely discussed. The percentage of third molars extracted for prophylactic reasons where there is not a justifiable clinical reason varies between %18 and %50. The defenders of the prophylactic removal approach argue that the removal of these non-functional teeth decreases the possibility of pathological changes in the future and incidence of mandibular fractures. Those opposing this approach advocate that the patients unnecessarily suffer an surgical trauma and it’s related complications; post-operative pain and expenditure.
