KATARAKT CERRAHİSİ SONRASI URRETS-ZAVALİA SENDROMU

Abstract

Urrets-Zavalia sendromu(UZS) ilk defa 1963 yılında keratokonuslu olgularda midriyatik kullanılarak yapılan penetran keratoplasti (PKP) sonrasında gelişen ve sekonder glokomun eşlik ettiği , atrofik dilate pupilla şeklinde ortaya çıkan bir olgu olarak tanımlanmıştır. Urrets-Zavalia sendromu en çok PKP sonrası geliştiği bildirilmekte ise de trabekülektomi ,derin anterior lamellar keratoplasti , desme soymalı endotelyal keratoplasti, katarakt cerraihisi, gonyotomi, fakik intraokuler lens implantasyonu yapılan olgular sonrasında da bildirilmiştir. En önemli risk faktörü ise cerrahi sırasında veya sonrasında yükselen intraokuler basınca sekonder gelişen iris iskemisidir fakat sorunsuz katarakt cerrahisi sonrasında da görülebilir. Tedavide midriyatik ajanların kullanımı ve pupilla rekonsriksiyonu kullanılabilir.
