Onkolojik Hastalarda Vitamin D Düzeyleri Ve Prognostik Etkisi

Abstract

Giriş: Kanser günümüzde hastalık kaynaklı mortalite istatistiklerinde kardiovasküler hastalıklardan sonra ikinci sırada gelmektedir. Bu çalışmada kliniğimizde tedavi alan kanser hastalarında vitamin D

düzeylerine ve onların prognostik etkisine baktık.

Metod: 2014-2015 tarihlerinde Tıbbi Onkoloji Kliniğimize ilk defa başvuran ve yeni kanser tanısı alarak tedavi planı yapılan hastalar çalışmaya dâhil edildi.

Bulgular: Çalışmaya alınan hastaların 236 hastanın vitamin D düzeyleri ölçülmüştü. Hastaların yaş ortalaması 60±12 olup 137 erkek (%58) 99 hasta kadın idi (%42). Evrelere göre ayrıldığında evre 1 kanser

44 hasta (%18,6), evre 2 kanser 81 hasta (%34,3), evre 3 kanser 33 hasta (%14) ve 78 hasta (%33) evre 4 kanserden oluşmaktaydı. Genel hasta popülasyonuna baktığımızda başlangıç vitamin

D düzeyleri ile genel sağkalım (OS) ve progresyonsuz sağkalım (PFS) arasında istatistiksel anlamlı bir ilişki saptanmamıştır (PFS için P=0.45, HR 0,98-1,036, OS için P=0.1, HR 0,99-1,045).

Ancak kolorektal kanser ile başlangıç vitamin D düzeyleri arasında anlamlı bir ilişki bulduk (p=0,027, HR 1.11).

Sonuç: Vitamin D düzeyleri kolorektal kanser tanısı olan hastalarda genel sağkalım ve progresyonsuz sağkalım ile ilişkili olarak bulundu, bu sonuçta koleraktal kanser tanılı hastalarda vitamin D

düzeylerinin takibi ve replasmanının kanser gidişatı ile daha yakın ilgili olabileceğini düşündürmektedir.
