Yoğun Bakım Hemşirelerinde Uykusuzluk ve Hasta Güvenliği: Riskler ve Çözüm Önerileri

Abstract

Yoğun bakım üniteleri, kritik hastaların izlenip tedavi edildiği özel birimlerdir. Bu birimlerde hemşirelerin hastaların vital bulgularını, monitörizasyon cihazlarını ve genel durumlarını dikkatle takip etmesi, hasta güvenliği açısından kritik öneme sahiptir. Gece çalışma şartlarının yoğunluğu, dikkat ve algı üzerindeki etkileri nedeniyle hemşire sağlığını ve hasta güvenliğini önemli ölçüde etkilemektedir. Araştırmalar, gece vardiyasında çalışan hemşirelerin %30’unun işte uyuyakaldığını, yorgunluk nedeniyle dikkat eksikliği yaşayan hemşirelerin hata yapma riskinin arttığını, haftada birden fazla gece vardiyasında çalışan gebe kadınların ertesi hafta spontan düşük yapma riskinin daha yüksek olduğunu göstermektedir. Vardiyalı ve uzun çalışma saatleri, hemşirelerde uyku bozuklukları, yorgunluk, dikkat dağınıklığı ve karar verme yetisinde azalma gibi sorunlara yol açmakta, bu durum ilaç hataları, tedavi uygulamalarında aksaklıklar ve hasta bakımında ciddi tehlikeler yaratmaktadır. Bu durum yalnızca hemşirelerin fiziksel ve psikolojik sağlıklarında olumsuz etkiler yaratmakla kalmayıp, hasta memnuniyetini ve bakım kalitesini de düşürmektedir. Bu derlemenin amacı yoğun bakımda çalışan hemşirelerin vardiyalı ve gece çalışma saatlerinin dikkat, algı ve uyku süreçlerine olan olumsuz etkileri, bu etkilerin hasta bakımında yaratabileceği risklerin neler olabileceğini göstermek ve bu sorunları azaltmaya yönelik çözüm önerileri sunmaktır. Ayrıca yoğun bakımda uykusuzluk yaşayan hemşirelerin, uyuyan hastaları fark etme kapasitesi üzerine odaklanmaktadır. Yoğun bakım ünitelerinde hasta ve hemşire arasındaki etkileşim, özellikle hemşirelerin uyku eksikliği ve dikkat seviyelerinin hasta bakımına etkisi açısından önem arz etmektedir.

Insomnia and Patient Safety in Intensive Care Nurses: Risks and Solution Suggestions

Abstract

Intensive care units are specialized units where critically ill patients are monitored and treated. In these units, it is critically important for nurses to carefully track patients’ vital signs, monitoring devices, and overall condition for ensure patient safety. The intensity of night shifts significantly affects both nurse well-being and patient safety due to its impact on attention and perception. Research shows that 30% of nurses working the night shift fall asleep at work, that nurses experiencing fatigue-induced inattention have an increased risk of making errors, and that pregnant women who work more than one night shift per week face a higher risk of spontaneous abortion in the following week. Shift work and long working hours lead to issues such as sleep disorders, fatigue, decreased concentration, and reduced decision-making abilities in nurses, which in turn can result in medication errors, disruptions in treatment protocols, and serious risks in patient care. This situation not only adversely affects the physical and psychological health of nurses but also lowers patient satisfaction and the overall quality of care. The aim of this review is to demonstrate the negative effects of shift work and night shifts on the attention, perception, and sleep processes of nurses working in intensive care, to identify the potential risks these effects may pose to patient care, and to propose solutions to mitigate these issues. Additionally, the review focuses on the capacity of nurses experiencing insomnia in intensive care to notice patients who are asleep. The interaction between patients and nurses in intensive care units is of paramount importance, particularly regarding how nurses’ sleep deprivation and attention levels can affect patient care.
