Parents’ knowledge, attitudes, and practices regarding the management of fever in children: a single-center cross-sectional study

Abstract

Aims: Fever is a frequent reason for pediatric healthcare visits, and parents’ misconceptions may lead to unnecessary antipyretic use and early emergency admissions. This study aimed to evaluate parents’ knowledge, thresholds for initiating antipyretics, home management practices, and factors associated with early treatment of fever. Methods: This cross-sectional study included 440 parents of children aged 0–12 years who presented to the Pediatric Outpatient Clinic of Malatya Training and Research Hospital between June and September 2025. Data were collected using a 40-item structured questionnaire. Categorical variables were analyzed with Chi-square tests, and predictors of early antipyretic use (≤38°C) were assessed through multivariate logistic regression. Results: Among participants, 76.1% were mothers and the mean age was 32.4±6.8 years. Although 92% recognized fever as a defense mechanism, 64.8% initiated antipyretics at ≤38°C. Lower education level, a history of febrile seizures, and the belief that fever is harmful were independently associated with early antipyretic use (p<0.01). Conclusion: Despite generally adequate knowledge, inappropriate fever management practices—especially early antipyretic use—remain common. Educational status and fear of febrile seizures are major determinants of these behaviors. Targeted parent education may reduce unnecessary drug use and emergency department visits.
