Aims: This study aimed to evaluate the prognostic performance of the age–male–albumin–bilirubin–platelet (aMAP) score in predicting 28-day mortality among patients with acute pancreatitis (AP) presenting to the emergency department (ED).
Methods: We retrospectively analyzed adult patients diagnosed with AP between January 2020 and December 2024 in a tertiary ED. Demographic characteristics, laboratory findings, clinical parameters, and mortality outcomes were recorded. aMAP scores were calculated using a validated formula. The prognostic performance of the aMAP score was compared with that of ALBI, BISAP, SOFA, and lactate levels using logistic regression and ROC curve analysis.
Results: A total of 386 patients were included in the analysis. The median age was significantly higher among non-survivors than survivors (77 vs. 60 years, p<0.001). Levels of aMAP, ALBI, BISAP, SOFA, and lactate were all significantly higher in the non survivor group (p<0.001 for all). In multivariate logistic regression analysis, aMAP (OR: 4.63; 95% CI: 1.50–14.32; p=0.008) and lactate (OR: 1.62; 95% CI: 1.12–2.33; p=0.010) emerged as independent predictors of mortality. ROC analysis demonstrated that lactate had the highest prognostic accuracy (AUC: 0.843), followed by aMAP (AUC: 0.821) and ALBI (AUC: 0.764).
Conclusion: These findings indicate that the aMAP score is a simple, accessible, and effective tool for early mortality risk stratification in patients with AP. Its ease of calculation and prognostic performance comparable to traditional scoring systems support its practical use in the emergency setting.
