Clinical characteristics, treatment patterns and survival outcomes of gynecological cancers: a single-center study from Çorum province

Abstract

Aims: This study aimed to evaluate the association between treatment approach, clinicopathological characteristics, tumor localization, FIGO stage, and survival outcomes in patients with gynecologic malignancies in a real-world, single-center cohort. Methods: This retrospective observational study included 196 patients diagnosed with gynecologic cancers between 2020 and 2025 at a tertiary referral center. Demographic, clinical, pathological, and survival data were collected from institutional records. Patients were analyzed according to tumor localization, histological subtype, menopausal status, comorbidity burden, FIGO stage, treatment modality, recurrence status, and overall survival. Statistical analyses included group comparisons and multivariable logistic regression to identify independent predictors of surgical treatment. Survival analysis was performed using appropriate comparative tests, and p<0.05 was considered statistically significant. Results: The mean age of the cohort was 61.18±11.78 years, and 80.1% of patients were postmenopausal. Endometrial cancer was the most common malignancy (63.8%), followed by ovarian (25.0%) and cervical cancers (8.7%). Surgical treatment was performed in 63.3% of patients. Treatment approach was significantly associated with tumor localization (p<0.001), with surgery predominating in ovarian cancer and non-surgical management more frequent in cervical cancer. FIGO stage was strongly associated with survival (p<0.001), with a marked decline in survival in advanced stages. However, no significant association was observed between treatment approach and overall survival. In multivariable analysis, menopausal status was the only independent factor associated with surgical treatment (OR=0.25, p=0.010), while age, comorbidity, and tumor location were not significant predictors. Conclusion: In gynecologic malignancies, tumor localization and FIGO stage are the primary determinants of treatment strategy and survival outcomes, respectively. While surgical treatment patterns vary according to cancer type, survival is predominantly driven by disease stage rather than patient-related factors such as age, comorbidity, or menopausal status. These findings highlight the central role of tumor biology and accurate staging in guiding clinical decision-making and prognostication.
