From juvenile bleeding to polycystic ovary syndrome: how adolescent gynecology shifts across age groups—a retrospective analysis of 9,656 visits

Abstract

Aims: To characterize the clinical profile of a tertiary adolescent gynecology outpatient clinic, describe age-related differences in the recorded spectrum of visits, and describe the temporal distribution of visits across school and vacation periods. Methods: This retrospective, registry-based study analyzed 9,656 outpatient visits by 8,362 unique female patients aged 10-19 years at a tertiary city hospital (January 2024–February 2026). Patients were stratified into early (10–13), middle (14–16), and late (17–19) adolescence. Diagnostic distributions across age and social status were compared using Chi-square (or Fisher's) tests with Benjamini Hochberg FDR correction and Bonferroni post-hoc tests. Multivariable logistic regression assessed independent age and social-status effects. Visit density between school and vacation periods was compared with Mann–Whitney U; linear temporal trend by regression. Results: Late adolescents accounted for 57.0% of visits, middle 31.8%, and early 11.2%. The leading visit outcomes were normal gynecological examination (28.3%), dysmenorrhea (12.0%), oligomenorrhea (11.0%), polycystic ovary syndrome (PCOS, 8.1%), and vaginitis (6.7%). After FDR correction, 13 of 20 diagnoses retained significant age-related differences. Juvenile bleeding showed a striking inverse age gradient (19.6%→2.7%→<0.1%; FDR-adjusted p<0.001); PCOS a positive gradient (0.7%→6.1%→10.6%). In multivariable logistic regression, late adolescents had markedly higher adjusted odds of PCOS (OR 16.11; 95% CI 7.96-32.61) and near-zero odds of juvenile bleeding. Visit density was higher during vacation (20.1±1.6) than school (17.4±2.9; p=0.055); linear temporal regression was non-significant (β=0.14; 95% CI≈0 to 0.29; p=0.056). Conclusion: Adolescent gynecology visits follow distinct, age-stratified patterns that persist after adjustment for social status. The high proportion of normal examinations and very low psychology referral rates highlight the need for accessible, developmentally tailored, and psychologically integrated care.
