Hemodiyaliz tedavisi gören kronik böbrek hastalığı olan hastalarda anemi ve sağlıkla ilgili yaşam kalitesi arasındaki ilişki

Abstract

Amaç: Anemi hemodiyaliz hastalarında sık görülen bir komplikasyondur ve kötü yaşam kalitesi (YK) ile ilişkilidir. Amacımız, hemodiyaliz tedavisi gören böbrek yetmezliği hastalarında anemi ve Sağlıkla İlişkili Yaşam Kalitesi (SİYK) arasındaki bağlantıyı incelemektir.
Yöntemler: Bu kesitsel çalışmaya hemodiyaliz tedavisi gören 128 kişi gönüllü olarak katıldı. Demografik, klinik ve laboratuvar parametreleri ve SF-12 kullanılarak YK değerlendirildi. Hemoglobin değerine göre anemisi olan ve olmayan hastaların sosyo-demografik özellikleri karşılaştırıldı. SF-Sağlık Anketinin fiziksel (PCS12) ve zihinsel alt ölçeklerinden (MCS12) elde edilen skorlar diğer değişkenlerle (yaş, cinsiyet, eğitim, sigara, medeni durum, komorbidite, BMI, hemodiyaliz süresi, hemoglobin değerleri) karşılaştırılmıştır. Regresyon analizleri PCS12 ve MCS12 bağımsız değişkenler olarak kullanılarak yapılmıştır.
Bulgular: Hb değerleri 10-11 arasında olan 27 (%21,1) hasta ve Hb değerleri 10'un altında olan 38 (%29,7) hasta vardı. Bu çalışmanın bulguları, erken yaş (p=0.001) ve yüksek eğitimin (p=0.009) daha iyi yaşam kalitesi ile bağlantılı olduğunu gösterirken, anemi ile herhangi bir ilişki bulunamamıştır (p=0.228). Sigara kullanımı da daha iyi skorlar için anlamlı bulunmuştur (p=0.01). Anemisi olan ve olmayan hastalar arasında sosyo-demografik özelliklerin karşılaştırılması, evli hastaların istatistiksel olarak anlamlı şekilde daha yüksek hemoglobin seviyeleri sergilediğini ortaya koymuştur (p=0.001).
Sonuç: SF-12 anketinin fiziksel boyutu ile hem genç yaş hem de yüksek eğitim düzeyi arasında kayda değer bir ilişki tespit edilirken, zihinsel boyut için böyle bir ilişki gözlenmemiştir. Bulgular, daha genç ve daha yüksek eğitim seviyesine sahip bireylerin daha iyi bir fiziksel YK yaşadıklarını göstermiştir. Bununla birlikte, anemi ve YK arasında bir bağlantı bulunamamıştır.

The relationship between anemia and health-related QoL in patients with chronic kidney disease receiving hemodialysis treatment

Abstract

Aims: Anemia is a frequent complication in hemodialysis patients and associated with a poor quality of life (QoL). Our goal was to examine the connection between anemia and health-related QoL (HR-QoL) in those with renal failure receiving hemodialysis treatment.
Methods: In this cross-sectional study, 128 subjects that underwent hemodialysis volunteered. Demographic, clinical, and laboratory parameters and QoL using SF-12 were assessed. Socio-demographic characteristics in patients with and without anemia according to hemoglobin value were compared. The scores obtained from the physical (PCS12) and mental subscales (MCS12) of the SF-health survey were compared with other variables (age, gender, education, smoking, marital status, comorbidity, BMI, duration of hemodialysis, hemoglobin values). Regression analyses were conducted using PCS12 and MCS12 as the independent variables.
Results: There were 27 (21.1%) patients with Hb values between 10-11 and 38 (29.7%) patients with Hb values below 10. The findings of this study indicated that an early age (p=0.001) and higher education (p=0.009) were linked to improved QoL, while no association was found with anemia (p=0.228). Smoking was also found to be significant (p=0.01) for better scores. Univariate linear regression analysis was performed to evaluate the interaction or relationship between age and smoking. Smoking was found to be an effective risk factor in patients younger than 65 years, but not in patients older than 65 years. The comparison of socio-demographic characteristics between patients with and without anemia revealed that married patients exhibited statistically significant higher hemoglobin levels (p=0.001).
Conclusion: A notable association was identified between the physical aspect of the SF-12 survey and both younger age and higher educational level, whereas no such relationship was observed for the mental aspect. The findings indicated that younger individuals with higher education levels experienced an improved physical QoL. However, no link has been found between anemia and QoL.

This study was conducted in accordance with the principles outlined in the Declaration of Helsinki. Informed consents of all patients were obtained before administration of the QoL scale. The study was approved by our institutional ethical review board (Date: 31.01.2024 Number: AEŞH-BADEK-2024-054).
