The relationship between the frequency and severity of postpartum diastasis recti abdominis with the density of striae gravidarum

Abstract

Aims: This study aims to evaluate how the presence and severity of striae gravidarum (SG) affect the frequency and severity of postpartum diastasis recti abdominis (none/mild/severe).
Methods: This study included 107 patients who underwent caesarean section for various indications and were examined for diastasis recti during postpartum outpatient clinic visits. SG was scored according to the Davey scoring system and interrectal distances were measured by ultrasonography during postpartum outpatient clinic visits and evaluated for diastasis recti at postpartum weeks 3-6.
Results: Patients who scored 0 points according to the Davey score, indicating the absence of striae, were excluded from the study. Patients in the mild and severe groups were then compared in terms of DRG 1 and DRG 2. It was observed that the groups were homogeneously distributed. A significant difference (p<0.001) was found in terms of the supra-umblic interrectal distance (DRG 1) between the mild striae group (Davey score 1–2) and the severe striae group (score ≥3). When evaluated in terms of subumbilical interrectal distance (DRG 2), a significant difference was observed between the groups (p=0.003). The findings indicate that increasing striae density correlates with greater interrectal distance, placing patients at higher risk of diastasis recti.
Conclusion: The SG score can be utilized as a predictive tool to determine the likelihood of diastasis recti occurring during pregnancy. Furthermore, this study could contribute to understanding the pathogenesis of both SG and diastasis recti by examining collagen and fascia tissue samples in larger patient populations.

Stria gravidarum yoğunluğu, postpartum diastazis rekti abdominis sıklık ve şiddetiyle ilişkili midir?

Abstract

ÖZET

AMAÇ: Disfonksiyonel fibroblastlar, hem Stria Gravidarum hem de Diastasis Rekti Abdominis histopatogenezinden sorumlu olabilecek ortak noktadır. Bu bilimsel gerçeklikten yola çıkarak; Stria Grivadarum varlığının, şiddetiyle (yok/hafif/ciddi) orantılı olarak, Postpartum Diastazis Rekti Abdominis’in sıklığını ve şiddetini nasıl etkilediğini değerlendirmek, çalışmamızın ana gerekçesini oluşturmaktadır

GEREÇ VE YÖNTEM: Bu çalışma Mart 2021-2022 tarihleri arasında kadın hastalıkları ve doğum servisine yatışı yapılan ve çeşitli nedenler ile sezeryan ile doğumu gerçekleştirilmiş, doğum sonrası poliklinik kontrollerinde diastazis rekti muayenesi de yapılan olguları kapsamaktadır. Gebelerde Davey skorlama sistemine göre stria gravidarum puanlaması yapılmış ve doğum sonrası 3-6. aylarda poliklinikte interrektal mesafeler ölçülerek diastazis rekti açısından değerlendirme yapılmıştır.

BULGULAR: Davey skoruna göre 0 puan alan yani striası bulunmayan hastalar dışarıda bırakılarak hafif ve ağır gruptaki hastalar dra1 ve dra 2 açısından karşılaştırıldı. Davey skoru 1-2 olan Hafif strialı grup ile skor 3 ve daha yüksek olan ağır strialı grup arasında umblikus üstü interrektal mesafe (dra1) açısından grupların homojen dağıldığı ve (p=0,001) anlamlı fark olduğu görüldü. Umblikus altı interrektal mesafe (dra2) açısından değerlendirildiğinde ise yine grupların homojen dağıldığı (p=0,014) ve gruplar arasında anlamlı fark olduğu görüldü. Bu verilerden yola çıkılarak stria yoğunluğu arttıkça interrektal mesafenin de artacağı ve diastazis rekti açısından hastanın daha riskli grupta yer alabileceği beklenmektedir.

SONUÇ: Gebelik döneminde oluşan diastazis rektinin kalıcı olup olmayacağını ön görmede stria gavidarumun skoru klavuz olarak görülebilir. Ayrıca bu çalışma daha geniş hasta gruplarında ve fasya doku örneklerinde çalışılan kollajen ile desteklendiğinde stria gravidarum ve diastazis rekti patogenezine ışık tutabilecektir.
