Hipertansiyonlu adolesanlarda uyku kalitesinin değerlendirilmesi

Abstract

Amaç: Bu araştırmanın amacı hipertansiyon tanılı adolesanlarda uyku kalitesinin değerlendirilmesi ve sonuçların aynı yaş grubundaki normotansif sağlıklı adolesanlar ile karşılaştırılmasıdır.
Yöntem: Gözlemsel vaka-kontrol çalışması olarak tasarlanan bu araştırmaya; 2023 – Ağustos 2023 tarihleri arasında,13-18 yaş arasında 80’ hasta, 80’i de sağlıklı toplam 160 adolesan dahil edildi. Yüzyüze görüşme yöntemi ile yapılan araştırmada katılımcılara, klinik ve sosyodemografik veri formu ile uyku kalitesinin belirlenmesi için Türkçe geçerlilik ve güvenilirlik çalışması yapılmış olan Pittsburgh Uyku Kalite İndeksi (PUKİ) ölçeği uygulandı. Veri ve testlerin değerlendirilmesinde istatistiksel anlamlılık düzeyi p ≤ 0,05 kabul edildi.
Bulgular: Araştırmaya katılan toplam 160 katılımcının %35’i (n=56) kız, %65’i (n=104) erkekti. Pittsburgh Uyku Kalite İndeksi (PUKİ) toplam skoru vaka grubunda ortalama 6,225 (±1,090), kontrol grubunda ortalama 4,287 (±1,264) olarak saptandı. Vaka grubundaki katılımcıların günlük ortalama uyku süresi 7,42 (±0,595) saat, kontrol grubundakilerin ise 7,61 (±0,539) saat saptandı. Benzer şekilde vaka grubundaki katılımcıların %72,5’inin (n=58), kontrol grubundakilerin ise %15’inin (n=12) PUKİ toplam skorunun 5’in üzerinde olduğu (kötü uyku kalitesi) görüldü. Vaka grubunda uyku kalitesi daha düşüktü. Vaka grubundaki katılımcıların %27,5’inin (n=22) Pittsburg Uyku Kalite İndeksi (PUKİ) toplam skorunun 5 ve 5’in altında (iyi uyku kalitesi), kontrol grubundakilerin de %85’inin (n=68) PUKİ toplam skorunun iyi uyku kalitesi olduğu görüldü.
Sonuç: Hipertansiyon tanısı alan adolesanlarda global PUKİ skoru ve bileşenlerinin daha yüksek olduğu ve hipertansiyonun kötü uyku kalitesi ile ilişkili olduğu sonucuna varıldı.

Evaluation of sleep quality in adolescents with hypertension

Abstract

Aims: This study aimed to evaluate sleep quality in adolescents diagnosed with hypertension and to compare the results with those of normotensive healthy adolescents of the same age group.
Methods: Designed as an observational case-control study, this research included 160 adolescents between the ages of 13-18, with 80 patients and 80 healthy controls, between February and August 2023. Data were collected through face-to-face interviews using a clinical and sociodemographic data form, and sleep quality was assessed with the Pittsburgh Sleep Quality Index (PSQI), which has been validated in Turkish. A significance level of p≤0.05 was accepted in the evaluation of data and tests.
Results: Among the 160 participants, 35% (n=56) were female and 65% (n=104) were male. The mean PSQI total score was 6.225 (±1.090) in the case group and 4.287 (±1.264) in the control group. The mean daily sleep duration was 7.42 (±0.595) hours in the case group and 7.61 (±0.539) hours in the control group. Similarly, 72.5% (n=58) of the case group and 15% (n=12) of the control group had a PSQI total score above 5 (poor sleep quality). Sleep quality was lower in the case group. Good sleep quality (PSQI≤5) was observed in 27.5% (n=22) of the hypertensive adolescents, while 85% (n=68) of the controls had good sleep quality.
Conclusion: It has been observed that adolescents diagnosed with hypertension have higher overall PSQI and component scores, thus hypertension is associated with poor sleep quality.
