Elli yaş üstü olgularda tarama kolonoskopisi sonuçlarımız

Abstract

According to the World Health Organization data, colorectal cancer is the 3rd most common form of cancer and ranks 3rd in mortality due to cancer death. The explanation of the genetics and pathophysiology in colorectal carcinogenesis has provided the standardization of the screening program in colon cancer today. Although methods such as occult blood in the stool are also used, the gold standard in this regard is colonoscopic examination. In this study, we aimed to present the results of patients who underwent screening lower gastroin-testinal tract examination within the indication. The data of the patients who underwent screening colonoscopy between January 2016 and January 2023 in our clinic were scanned from the Nucleus database. Age, gender, other demographic findings, postopera-tive complications, and pathological findings of the patients were recorded. Colonoscopy was performed in 1992 patients aged 50 and over in our clinic. The reports of these pati-ents were reviewed. Normal colonoscopic findings were observed in 701 (32.7%) patients who underwent scree-ning colonoscopy. Of the cases, 553 (25.8%) had polyps, 99 (4.6%) masses, 219 (10.2%) diverticula, 8 (0.4%) patients had angiodysplasia, 99 (4.6%) colitis, 13 (0.6%) ulcers, 10 (0.5%) rectitis, 274 (12.8%) hemorrhoids, and 169 (7%) inadequate bowel cleansing. After gaining the necessary experience in endoscopic procedures, we believe that this procedure is a survival-enhancing method, especially with the detection of precancerous lesions in the early stage of colorectal cancer, with the increase in the number of centers where the procedure can be performed by completing the learning curve in colonoscopy and the spread of screening programs.

Abstract

Dünya Sağlık Örgütü verilerine göre kolorektal kanserler en sık görülen 3. kanser şekli olup, mortalitede kanser ölümüne bağlı ölümlerde 3. sıradadır. Kolorektal karsinogeneziste genetik ve patofizyolojinin açıklan-ması, günümüzde kolon kanserinde tarama programının standardizasyonunu sağlamıştır. Her ne kadar gaitada gizli kan gibi yöntemler de kullanılıyor olsa da, bu konudaki altın standart kolonoskopik incelemedir. Biz bu çalışmada, endikasyon dahilinde tarama alt gastrointestinal sistem incelemesi yaptığımız hastaların sonuçlarını sunmayı amaçladık. Kliniğimizde Ocak 2016-Ocak 2023 tarihleri arasında tarama amaçlı kolonoskopi incelemesi yapılan olguların verileri Nucleus veritabanından tarandı. Hastalara ait yaş, cinsiyet, diğer demografik bulgular, işlem sonrası komplikasyonlar, saptanan patolojik bulgular kaydedildi. Kliniğimizde 50 yaş ve üstü 1992 olguya kolonoskopi yapıldığı görüldü. Bu hastaların raporları ince-lendi. Tarama amaçlı kolonoskopi uygulanan 701 (%32,7) olguda normal kolonoskopik bulgular izlendi. Olgu-ların 553’ünde (%25,8) polip, 99’unda (%4.6) kitle, 219’unda (%10,2) divertikül, 8’inde (%0.4) hastada an-jiodisplazi, 99’unda (%4,6) kolit, 13’ünde (%0,6) ülser, 10’unda (%0,5) rektit, 274’ünde (%12,8) hemoroid ve 169’unda (%7,9) yetersiz barsak temizliği görüldü. Endoskopik işlemlerde gerekli deneyimi kazandıktan sonra, kolonoskopide öğrenme eğrisini tamamla-yarak işlem yapılabilen merkez sayısının artması ve tarama programlarının yaygınlaştırılmasıyla, kolorektal kanserde erken evrede özellikle prekanseröz lezyonların saptanması ile bu işlemin sağkalım artırıcı bir yöntem olduğu kanaatindeyiz.
