EMERGENCY THYROIDECTOMY

Abstract

A 46-year old male patient was brought in because of the heavy obstruction of the upper airway due to intra-nodular bleeding of neglected benign huge multinodular goiter, dyspnea, and hypoxia. After the rapid increase of pCO2 levels and slight loss of consciousness, the patient was anesthetized immediately and thyroidectomy was suggested before, because of tracheotomy technically difficult to ensure the patient's airway risk due to the fact that of an emergency operation was performed. The general condition of the patient to provide the airway with the aim of provide total thyroidectomy and than tracheotomy was performed. Postoperative bleeding, wound infection and hipoparathyroidizm were not seen. Postoperative vocal cord movement was normal. This was an uncommon situation, the fact that provides the airway speciality and airway management. Here in, we discussed the patient in view of the literature.

Acil Tiroidektomi

Abstract

46 yaşındaki erkek hasta, son beş-altı saatte boyunda ağrı ve şişliğin artması, şiddetli üst solunum yolu obstrüksiyonu, dispne, siyanoz ve hipoksi nedeniyle acil servise getirildi. Öncesinde ihmal edilmiş büyük multinodüler guatr’ın intra nodüler kanamasına bağlı, trakeal bası sonucu akciğerde karbondioksit retansiyonu ve bilincinin bozulması üzerine acil serviste entübe edildi. Hastanın hava yolunun sağlanması için trakeotominin tiroidektomi öncesi yapılmasının riskli ve teknik olarak zor olması nedeniyle acil tiroidektomi operasyonu planlandı. Hastanın havayolunun sağlanması ve genel durumunun düzeltilmesi amacı ile acil total tiroidektomi ve trakeotomi yapıldı. Postoperatif kanama, yara yeri enfeksiyonu ve hipoparatiroidizm görülmedi. Postoperatif vokal kord hareketleri doğaldı. Nadir olması ve havayolunun sağlanmasında özellik taşıması nedeni ile rapor edildi. Vaka literatür bilgileri ışığında tartışıldı.
