COMPARISION OF THE USAGE OF SLIT AND NON-SLIT MESHES IN LAPAROSCOPIC TOTAL EXTRAPERITONEAL HERNIA REPAIR

Abstract

The inguinal hernia repair is one of the most commonly performed operations in general surgery. The different types of meshes were used in total extra-peritoneal (TEP) inguinal hernia repair. However, there is still contraversion about the most widely used slit and non-slit meshes. The aim of this study was to determine the differences of the usage of slit and non-slit meshes. We compared the both techniques in terms of postoperative complications and operative time. In our retrospective study between January 2009 and December 2010, a total of 236 laparoscopic inguinal hernia repair was performed in 195 male patients. A total of 54 slit mesh was used in 43 patients. On the other hand a total of 182 non-slit mesh was used in 152 patients. Hernia recurrence was seen in 4 (2,19%) patients in the non-slit mesh group however there was no recurrence in the slit mesh group (p <0.05). There was seroma development in 10 (5,49%) patients in non-slit mesh group and 3 (5,55%) patients in slit mesh group (p>0.05). Discussions continue on the type of mesh used. In our study, the use of slit mesh in total extraperitoneal herni repair is safe and we found that in the term of hernia recurrence slit mesh has advantage over non-slit mesh. There was no difference in operative time and complications.

. Laparoskopik total ekstra peritoneal (TEP) inguinal fıtık onarımı yapılan hastalarda bacaklı ve düz mesh kullanımının karşılaştırılması.

Abstract

İnguinal fıtık onarımı genel cerrahide en sık yapılan ameliyatlardan biridir. Total ekstra peritoneal (TEP) inguinal fıtık onarımında değişik mesh çeşitleri kullanılmıştır. Ancak en çok kullanılan bacaklı mesh ve düz mesh kullanımı ile ilgili halen tartışma devam etmektedir. Çalışmamızın amacı bacaklı ve düz mesh kullanımı arasındaki farkları ortaya koymaktır. Her iki tekniği nüks, postoperatif komplikasyonlar ve ameliyat süresi açısından karşılaştırdık. Retrospektif çalışmamızda Ocak 2009 ile Aralık 2010 tarihleri arasında 195 erkek hastaya toplam 236 laparoskopik TEP inguinal fıtık onarımı gerçekleştirildi. 43 hastada 54 ameliyatta bacaklı mesh kullanıldı. 152 hastada ise 182 ameliyatta düz mesh kullanıldı. Fıtık nüksü düz mesh kullanılan grupta 4 (% 2.19) hastada görülürken bacaklı mesh kullanılan grupta görülmedi (p<0.05). Düz mesh konulan 10 (%5.49) hastada ve bacaklı mesh konulan 3 (%5.55) hastada seroma gelişimi mevcuttu (p>0.05). Kullanılan mesh tipi ile ilgili tartışmalar devam etmektedir. Biz çalışmamızda, total ekstraperitoneal fıtık onarımında bacaklı mesh kullanımı güvenli olduğunu ve fıtık nüksü göz önüne alındığında düz meshe göre daha avantajlı olduğunu saptadık. Operasyon zamanı ve komplikasyonlar açısından herhangi bir fark saptanmamıştır.
