Caecal endometriosis mimicking acute appendicitis. Case report.

Abstract

Presence of endometrial glands and stroma outside the intrauterine cavity is defined as endometriosis. It’s a benign disease which is common in childbearing women. Clinical presentation of intestinal endometriosis is usually asymptomatic. However, it may present with symptoms such as intestinal bleeding, nausea, vomiting, abdominal pain and cramp, painful defecation, diarrhea, constipation and recto-vaginal mass. It may also present with complications such as intussusception, intestinal obstruction and intestinal perforation. In generally, the most common cause of surgery for patients admitted to the emergency department because of right lower quadrant pain is acute appendicitis. Caecal and appendicular localization of endometriosis is a rare condition. Preoperative diagnosis is difficult, but it should be considered in women of childbearing age in the differential diagnosis not dependent on radiological imaging. We aimed to present a 22-year-old female patient with caecal endometriosis mimicking acute appendicitis prior asymptomatic in this case report.

AKUT APANDİSİTİ TAKLİT EDEN ÇEKAL ENDOMETRİOZİS OLGUSU.

Abstract

Rahim içi boşluğun dışında endometrial bezlerin ve stromanın varlığı endometriozis olarak tanımlanır. Doğurganlık çağındaki kadınlarda yaygın olarak görülen benign bir hastalıktır. İntestinal endometriozisin klinik prezentasyonu genellikle asemptomatiktir. Ancak intestinal kanama, bulantı, kusma, kramp tarzı karın ağrısı, ağrılı defekasyon, ishal, kabızlık, rekto-vajinal kitle gibi semptomlar; intussepsiyon, bağırsak obstrüksiyonu ve intestinal perforasyon gibi komplikasyonlarla da prezente olabilir. Genel olarak sağ alt kadran ağrısı nedeniyle acil servise başvuran ve akut abdomen saptanan hastalara yönelik cerrahi girişimlerin en sık nedeni akut apandisittir. Çekal ve apendiks lokalizasyonlu endometriozis nadir görülen bir durumdur. Preoperatif tanısı zordur ancak akut apandisit semptomları gösteren doğurganlık çağındaki kadınlarda, görüntüleme yöntemlerine bağlı kalmadan, ayrıcı tanıda göz önünde bulundurulması gerekir. Bu olgumuzda doğurganlık çağında olan ve daha öncesinde asemptomatik olan, akut apandisiti taklit ederek prezentasyon gösteren ve cerrahi sırasında çekal endometriozis saptanan 22 yaşında bir kadın hastayı sunmayı amaçladık.
