Gastrokolik fistül gastrointestinal sistem malign tümörlerinin nadir rastlanan bir komplikasyonudur. Batı ülkelerinde, kolon ve mide kanserleri maligniteye bağlı gastrokolik fistüllerde en sık karşılaşılan sebeplerdir. Bu yazıda 78 yaşındaki bir erkek hastamızda klasik gastrokolik fistülü semptom triadını gösteren bir vakamızı sunduk. Hastaya tümörlü bölgeyi unblok çıkaracak şekilde sol hemikolektomi, distal pankreatektomi, subtotal gastrektomi ve splenektomi uygulandı. Hasta postoperatif sorunsuz izlendi ve Medikal Onkoloji birimine refere edildi. Mide adenokarsinomları, gastrokolik fistüllerin nadir ancak önemli nedenlerinden biri olup iyi bir anamnez ve fizik muayene ile endoskopi ve baryumlu grafi yapılması tanı ve tedavi için önemlidir.
Gastrocolic fistula is a rare presentation of malignant diseases of the gastrointestinal tract. Malignant gastrocolic fistula is most commonly associated with adenocarcinoma of the transverse colon and gastric carcinom in the Western World. We report a case of a 78-year-old man who presented with the classic triad of symptoms (including diarrhea, epigastric pain and vomiting) associated with gastrocolic fistula. Radical en-bloc surgery (including extended left hemicolectomy, distal pancreastectomy, subtotal gastrectomy, and splenectomy and than adjuvant chemotherapy were performed. Gastric adenocarcinomas are a rare but important cause of gastrocolic fistulas. History, physical and endoscopic examination and barium meal and enema are important for diagnosis and treatment.
