OUR RESULTS IN SURGICAL TREATMENT OF THE ESOPHAGEAL CANCERS

Abstract

Background: We reported the surgical outcome of 86 cases with esophageal cancer that we performed surgery in a period of 9 years. Methods: We operated eighty-seven cases with esophageal cancer at Yüzüncü Yıl University, School of Medicine, Department of Thoracic Surgery between June 1996 and June 2005 (47 female and 39 male patients). Median age was 51 (range, 25 to 81 years old) years old. The localization of the tumors were pharyngoesophageal junction in 1 case, middle thoracic esopha-gus in 25 cases, lower thoracic esophagus and cardia in 59 cases, and multicentric in 1 case. A surgical approach was performed through laparotomy and right thoracotomy in 53 ca-ses, right thoracotomy plus laparotomy and servical incision in 16 cases, transhiatal in 3 cases, and right thoracotomy in 8 cases and laparotomy in 6 cases. Results: Resection was performed in 72 cases. There was not peroperative mortality. In postoperative period, 17 patients died because of leakage of anastomosis (5) and cardiopulmonary complications (12). In pathological staging 26 patients were stage IIa, 8 patients were stage IIb, 43 patients were stage III and 9 patients were stage IV. As we pay attention the fol-low-up 8 alive and 13 deceased patients, the survival time changes between 2 months- 6 years and 8 months. Mean time was 33.4 months. Five years survival rate was 12% in stage IIa. Conclusions: In esophagus carcinoma, the most effective treatment is surgery both for curative or palliative intention. But survival rate of high stage patients is poor. Adjuvant therapy is needed.

ÖZOFAGUS KANSERLERĐNDE CERRAHĐ TEDAVĐ SONUÇLARIMIZ

Abstract

Amaç: Dokuz yıllık süre içerisinde cerrahi uyguladığımız 86 özofagus kanserli olgudaki sonuçları irdeledik. Materyal ve Metod: Haziran 1996 – Haziran 2005 tarihleri arasında Yüzüncü Yıl Üniversitesi Tıp Fakültesi Göğüs Cer-rahisi Anabilim Dalı’nda, 47’si kadın, 39’u erkek olmak üzere 86 özofagus kanserli olgu opere edildi. Ortalama yaş 51 (25-81) idi. Tümör 1 olguda faringoözofageal bileşke, 25 olguda orta torasik özofagus, 59 olguda alt torasik özofagus + kardiya’da lokalize olup 1 olguda da multisentrik idi. Elli üç olguda sağ torakotomi + laparotomi, 16 olguda sağ torakotomi + laparotomi + servikal kesi, 3 olguda transhiyatal, 8 olguda sağ torakotomi ve 6 olguda laparotomi ile yaklaşım uygulandı. Bulgular: Yetmiş iki olguda rezeksiyon uygulandı. Peropera-tif mortalite gözlenmedi. Postoperatif dönemde 5 olgu anasto-moz kaçağı, 12 olgu kardiyopulmoner sebepler olmak üzere 17 olgu kaybedildi. Patolojik evrelendirmede 26 olgu evre IIa, 8 olgu evre IIb, 43 olgu evre III ve 9 olgu evre IV idi. Takipte olan 8 sağ ve vefat eden 13 olgu dikkate alındığında sağkalım süreleri 2 ay ile 7 yıl 6 ay arasında değişmekte olup ortalama 33.4 aydır. Evre IIa olgularımızda 5 yıllık sağkalım %12’dir. Sonuç: Özofagus kanserinde, küratif veya palyatif de olsa en etkili tedavi hala cerrahidir. Fakat ileri evrelerdeki hastalarda sağkalım oranları oldukça düşüktür ve adjuvan tedavi gereklidir.
