MANAGEMENT OF A PATIENT WITH MOEBIUS SYNDROME; CASE REPORT.

Abstract

Moebius syndrome is a rare congenital displasia that VI (Nerves Abducens) and VII (Nerves Facialis) nerves are commonly affected unilateral or bilaterally. Occasionaly V,X,XI, XII cranial nerves are involved, resulting in difficulty of chewing, swallowing and caughing which often leads to respiratory complications. These symptoms are often known and threatened in childhood period. Anesthetic management is so important in these patients because of the difficulties in intubation caused by orofacial defects. In our case we reported an adult patient with maxillofacial defects that could cause difficult intubation who had no history of operation before and we presented our anesthetic management in this patient.

Abstract

Moebius sendromu (MS), temel olarak VI.(abducens) ve VII. (Facial) kranial sinirlerin tek veya çift taraflı etkilendiği nadir görülen bir hastalıktır. Sıklıkla diğer kraniyal sinirlerin de (V, X, XI, XII) tutulumu çiğneme, yutma güçlüğü ve solunumsal komplikasyonlara yol açabilen öksürme güçlüğüne yol açabilir. Bu semptomlar daha çok çocukluk çağında tanı ve tedavi imkanı oluşturur. Orofacial anomaliler dolayısıyla zor entübasyona neden olabilen bir sendrom olmasından dolayı anestezi yönetimi açısından da önem arz etmektedir. Olgumuzda; daha önce operasyon öyküsü olmayan yetişkin bir hastanın, belirgin maxillofacial deformitesi sebebiyle zor entübasyon olabileceği öngörülüp, bu hastadaki anestezi yönetimi sunuldu.
