MINIMALLY INVASIVE SURGICAL TECHNIQUES IN THE TREATMENT OF ACUTE PANCREATITIS.

Abstract

Pancreatitis often leads to complications such as sepsis, shock, renal and respiratory failure; therefore, the morbidity and mortality of the disease is increasing. Bile duct stones are responsible from the etiology of acute pancreatitis for 80% to 90%. Trauma, drug use, infections, toxins, heredity is responsible for the rate of 10-20%. With advances in minimally invasive surgical techniques, this techniques are used in selected and suitable cases in the treatment of pancreatitis local complications. In addition in patients who are considered conventional surgical treatment can not be tolerated minimally invasive surgery can be used. In patients that early diagnosis of acute pancreatitis under intensive care conditions can be treated successfully with antibiotherapy. However in patients sterile pancreatic necrosis occurred medical treatment may not be sufficient. As a minimally invasive approach percutaneous drainage,endoscopic drainage, video assisted retroperitoneal debridment and laparoscopic transperitoneal debridment can be performed.

Akut pankreatit tedavisinde minimal invaziv cerrahi teknikler

Abstract

Pankreatit sıklıkla sepsis, şok, böbrek ve solunum yetmezliği gibi komplikasyonlara yol açar; bu nedenle hastalığın morbidite ve mortalitesi artmaktadır. Safra yolu taşları akut pankreatit etyolojisinin %80-90’ından sorumludur. Travma, ilaç kullanımı, enfeksiyonlar, toksinler ve heredite ise %10-20 oranında sorumludur. Minimal invaziv cerrahi girişimlerdeki gelişmelerle birlikte pankreatitin lokal komplikasyonlarının tedavisinde bu tekniklerin kullanımı uygun ve seçilmiş vakalarda kullanılmaktadır. Ayrıca konvansiyonel cerrahi tedaviyi tolere edemeyeceği düşünülen hastalarda minimal invaziv cerrahi teknikleri kullanılabilmektedir. Erken dönemde teşhis edilen akut pankreatitli hastalarda yoğun bakım şartlarında ve uygun antibiyoterapi ile başarı bir şekilde tedavi edilebilmektedir. Ancak steril pankreatik nekroz gelişmiş hastalarda medikal tedavi yeterli olmayabilmektedir. Minimal invaziv yaklaşım olarak perkutan drenaj, endoskopik drenaj, video yardımlı retroperitoneal debridment ve laparoskopik transperitoneal debridman uygulanabilmektedir.
