Planning home health care services: vehicle routing and crew scheduling

Abstract

In the changing and developing world, the population is aging due to increasing population and prolonging average life expectancy. The ageing population also brings disease, the need for care and increases the burden on the health system. For this reason, Home Health Care (HHC) are offered for elderly/sick, needy of care individuals. HHC cover a variety of healthcare services offered by healthcare professionals by visiting patients at their homes. These services should be planned effectively and efficiently due to the increase patient demand and limited resources. In this context, two main problems are encountered: Assignment and routing. In the study, a visit plan for 11 personnel and 138 patients from different locations was created with the real data obtained from Ankara Eğitim Araştırma Hospital HHC Unit. First, teams were created from personnel with different qualifications in order to minimize the workload in Model-1. Later, in the Model-2, routes were determined in order to minimize the total distance traveled by each created team. These integer linear models created were solved with IBM Ilog Cplex Optimization Studio program. As a result, when comparing the current situation with the appropriate solution obtained, 18% improvement was achieved in the routes without providing extra personnel.

Evde sağlık hizmetlerinin planlanması: araç rotalama ve ekip çizelgeleme

Abstract

Değişen ve gelişen dünyada, artan nüfus ve ortalama yaşam süresinin uzaması sebebiyle nüfus yaşlanmaktadır. Yaşlanan nüfus da hastalığı, bakım ihtiyacını beraberinde getirmekte ve sağlık sistemi üzerindeki yükü artırmaktadır. Bu nedenle yaşlı/hasta, bakıma muhtaç bireyler için Evde Sağlık Hizmetleri (ESH) sunulmaktadır. ESH, sağlık personellerinin hastaları evlerinde ziyaret ederek sundukları çeşitli sağlık hizmetlerini kapsamaktadır. Bu hizmetler, hasta taleplerinin artması ve kaynakların sınırlı olmasından dolayı etkin ve verimli bir şekilde planlanmalıdır. Bu bağlamda iki temel problem ile karşılaşılmaktadır: Atama ve rotalama. Yapılan çalışmada Ankara Eğitim Araştırma Hastanesi ESH Birimi’nden alınan gerçek verilerle 11 personel ve farklı konumlarda bulunan, 138 hastanın ziyaret planı oluşturulmuştur. İlk olarak Model-1’de iş yükünün minimize edilmesi amacıyla, farklı niteliklere sahip personelden ekipler oluşturulmuştur. Daha sonra Model-2’de, oluşturulan her ekibin toplam kat ettiği mesafenin minimize etmesi amacıyla, rotalar belirlenmiştir. Oluşturulan bu tam sayılı doğrusal modeller, IBM Ilog Cplex Optimization Studio programı ile çözülmüştür. Sonuç olarak, elde edilen uygun çözüm ile mevcut durum karşılaştırıldığında ekstra personel sağlanmadan rotalarda %18 iyileştirme sağlanmıştır.
