Kendileri Ya Da Partner/Eşleri HIV Pozitif Olan Bireylerin Doğurganlık Niyetleri ve Etkileyen Faktörler

Abstract

ÖZ
Amaç:
Bu araştırma; HIV ile yaşayan bireylerin doğurganlık niyetleri ve etkileyen faktörleri belirlemeye yönelik yapılmış çalışmaları sistematik olarak incelemek amacıyla yapıldı.
Yöntem:
Çalışma 1 Haziran -1 Temmuz 2022 tarihleri arasında arama motorlarında uluslararası veri tabanı Pub-med kontrol edilerek yürütüldü. Araştırmada “HIV and Fertility” anahtar kelimesi kullanıldı. Araştırmaya dâhil etme kriterlerine uyan toplam 38 yayın çalışma kapsamına alınarak karşılaştırma, sınırlılık ve sonuçlar açısından değerlendirildi. Araştırma kapsamına alınan çalışmalar seçilirken; 2013-2022 yılları arasında yapılmış olan çalışmalar incelenmiş olup, İngilizce yayınlar tarandı, nitel, nicel, kesitsel tanımlayıcı çalışmalar, randomize kesitsel çalışmalar ve kohort çalışmaları dahil edildi.
Bulgular:
Bu sistematik derlemede HIV ile yaşayan bireylerin ve partnerlerinin/eşlerinin doğurganlık niyetlerinin çeşitli faktörlerden etkilense de tahmin edilenden fazla olduğu belirlenmiştir. Partnerlerden biri ya da her ikisi de HIV pozitif olan çiftlerin doğurganlık niyetinin en çok eş isteği, toplumsal olarak kişilere biçilen kadın erkek rolleri, soyu devam ettirme, gibi etkenlere bağlı olarak değiştiği görülmektedir. Eşlerin yaşının genç olması, başka çocuklarının ya da ortak çocuklarının olmaması, farklı cinsiyette çocuk isteme, ART tedavisi ile sağlıklı bebek sahibi olabilme imkânı, sağlıklıyken çocuk sahibi olma isteği gibi etkenler de doğurganlık niyetini olumlu yönde etkilemiştir. Ancak bazı çalışmalarda çiftlerin ART’nin bebeğe zarar vermesinden, partnere ya da bebeğe HIV bulaşmasından korktukları için doğurganlık isteklerinin azaldığı bildirilmiştir.
Sonuç:
HIV ile yaşayan bireylerin çocuk sahibi olma isteği bulunmaktadır. Bu nedenle HIV ile yaşarken çocuk sahibi olmak isteyen çiftlerin doğurganlık niyetinin göz ardı edilmemesi, sağlık profesyonelleri tarafından gerekli danışmanlık hizmetinin verilmesi çok önemlidir.
