Left Coronary Artery-Pulmonary Artery Fistula in Coronary Artery Disease

Abstract

Coroner artery fistula which is rare and congenital or acquired arise from whole coroner artery drainage all of cardiac chamber and great artery. Although coroner artery fistula is uncommon, it can cause significant mortality and morbidity rates. The article is presented which coroner artery disease and coroner-pulmonary artery fistula was accomplishment committed. Sixty three year-old male patient admitted to the hospital with chest pain. Non-ST myocardial infarction was diagnosed in the examinations. After coroner angiography, it was found coronary artery disease in addition between LAD proximal portion and main pulmonary artery fistula. Fistula repair and coronary bypass were performed successfully under cardiopulmonary bypass. Without hemodynamic problem in intensive care and service follow-up, the patient was discharged from the hospital in the seventh postoperative day. We think that surgical treatment of coronary fistulas in patients with coronary artery lesion is done at the earliest time would enable improvement in mortality and morbidity rates.

Koroner Arter Hastalığına Eşlik Eden Sol Koroner Arter-Pulmoner Arter Fistülü

Abstract

Koroner arter fistülü tüm koroner arterlerden kaynaklanabilen ve tüm kardiyak boşluklara ve büyük damarlara drene olabilen nadir görülen konjenital veya kazanılmış anomalilerdir. Nadir görülmelerine rağmen önemli oranda mortalite ve morbiditeye neden olabilirler. Bu yazıda başarılı şekilde koroner arter baypas ile birlikte koroner-pulmoner arter fistül onarımı yapılan olgu sunulmuştur. Altmış üç yaşındaki erkek hasta göğüs ağrısı şikayetiyle hastaneye başvurdu. Yapılan tetkiklerde non-ST miyokart enfarktüsü tanısı konuldu. Koroner anjiyografi sonrası koroner arter lezyonlarına ek olarak sol ön inen koroner arter proksimal segmenti ile ana pulmoner arter arasında fistül saptandı. Hastaya kardiyopulmoner baypas altında fistül onarımı ve koroner baypas operasyonu başarıyla yapıldı. Yoğun bakım ve servis takiplerinde hemodinamik problemi olmayan hasta postoperatif yedinci günde taburcu edildi. Koroner lezyonu olan hastalarda koroner fistüllerin cerrahi tedavisinin en erken zamanda yapılmasının, mortalite ve morbidite oranlarında iyileşme sağlayacağını düşünmekteyiz.
