DIAGNOSTİC DIFFICULTY DURING THE TURMOIL OF ADOLESCENCE- A CASE REPORT

Abstract

Psychotic and affective disorders may appear either suddenly or slowly with some prodromal symptoms such as social/ emotional withdrawal, affective changes, subtreshold level of odd perception and/or cognition. The prepsychotic period with prodromal symptoms often overlap with adolesence which has its own special cognitive and affective changes. Therefore, clinical presentation and diagnostic process become more complicated. In this case report, an adolescent patient who was admitted to the service with the preliminary diagnoses of Bipolar Disorder, Depressive Attack and Attention Deficit Hyperactivity Disorder according to DSM-5 classification and psychodynamically thought to be in an identity crisis was discussed.The prodromal symptoms of the patient were carefully assessed and his functionality increased significantly by the diagnosis and treatment of comorbid disorders. At the same time, psychosocial interventions, group and individual psychotherapies and occupational therapies helped the identity development of the adolescent. It should be remembered that prediagnostic process and biopsychosocial assessment with detailed investigation may highly change the whole life of patients
