BEYAZ CEVHER HİPERİNTENSİTELERİ VE DEPRESYONA EŞLİK EDEN SOMATİK BELİRTİLERİN İLİŞKİSİ

Abstract

Kranial manyetik rezonans görüntülemenin (MRG) T2 ve FLAIR (Fluid-attenuated inverse recovery) sekanslarında gözlemlenen beyaz cevher hiperintensiteleri (BCH) depresyon, bipolar bozukluk, obsesif kompulsif bozukluk gibi bir çok psikiyatrik hastalıkta gözlenmektedir Bu çalışmada psikiyatrik tanıdan bağımsız depresyona somatik belirtilerin eşlik ettiği hastalar ile somatik belirtilerin eşlik etmediği hastaları BCH açısından karşılaştırmak amaçlanmıştır. Çalışmanın hipotezi BCH sıklığı somatik belirtilerin eşlik ettiği depresif belirtisi olan hastalarda, somatik belirtilerin eşlik etmediği hastalara göre daha yüksektir.
Çalışmaya bir üniversite hastanesinin psikiyatri bölümü psikosomatik ünitesinde Ocak 2017-Haziran 2019 tarihleri arasında yatarak tedavi alan hastalar dahil edilmiştir. Herhangi bir beyin bölgesinde BCH varlığına göre hastalar BCH+(n=27) ve BCH- (n=53) olarak işaretlenmiştir. Bir sonraki aşamada kranial MRG’de beyin bölgesi incelemesi detaylandırılıp frontal BCH varlığına göre hastaları FBCH+(n=24) ve FBCH- (n=56) olarak ayrıca işaretlenmiştir. Bir sonraki aşamada SB+ ve SB- olarak ikiye ayrılmış hasta grubu hem BCH hem de FBCH açısından karşılaştırılmıştır. Sonrasında FBCH sayısı açısından SB+ ve SB- grupları karşılaştırılmıştır.
SB+ ve SB- grupları arasında BCH ve FBCH sıklığı açısından anlamlı fark yoktur (sırasıyla p=0,478; 0,881). FBCH'nin sayısı açısından SB+ ve SB- grupları arasında fark bulunamamıştır(p=0,926).
