Anxiety in COPD patient population during the COVID-19 Pandemic; should we consider tele-medicine?

Abstract

Objective: This cross-sectional study investigated the concerns of the COPD population towards the COVID-19 pandemic and their adaptation to changes in healthcare delivery.
Method: The study population consists of 100 patients who have been followed up with a diagnosis of COPD for at least one year. Patients answered questions about their demographic characteristics, smoking and exercise habits, COPD attacks, drug use, and hospital admissions. Pandemic concerns were evaluated with the newly introduced Coronavirus Anxiety Scale (CAS) in the literature.
Results: Geriatric patients percent age was 64% and 80% was married. Half of the patient population was male and the rate of patients who have regular income was 92%. Smoking parameters improved by 15%. Exercise behaviors did not change. Healthcare accessability did not change for 43% of patients, 49% reported a hesitation in getting an outpatient clinic application, whichled %29 to recieve healthcare on phone. Drug use did not change in 68% and 29% reported much more regular drug use when compared before pandemics. Attack frequency did not change in %43, while 37% reported fewer attacks.There was no change in attacks in 43 patients, 37 patients had fewer attacks. Less hospitalized COPD patients were 49%. Mean CAS was 1.72. There was no significant relationship in CAS averages.
Conclusion: During the pandemic period, patients suffered less damage with better protection and developed strong anxiety control. Nearly half of the patients avoided applying to the hospital to receive health services during the pandemic and used the alternative of receiving health care by telephone. With tele-medicine applications to be designed in detail in health policies, hospitalizations can be reduced and disease management can be facilitated.

KOAH Popülasyonunda COVID-19 Pandemi Kaygısı; Tele-Tıbbı Düşünmeli miyiz?

Abstract

Amaç: Bu kesitsel çalışma ile KOAH popülasyonunun COVID-19 pandemisine yönelik kaygıları ve sağlık hizmet sunumundaki değişikliklere adaptasyonları araştırılmıştır.
Yöntem: Çalışma evrenini en az bir yıldır KOAH tanısı ile takipli olan 100 hasta oluşturmaktadır. Hastalar demografik özellikleri yanısıra sigara ve egzersiz alışkanlıkları, KOAH atak, ilaç kullanım ve hastane başvurularına yönelik soruları cevaplamışlardır. Pandemiye ait kaygıları, literatüre yeni tanıtılan koronavirüs kaygı ölçeği (CAS) ile değerlendirilmiştir.
Bulgular: Hasta populasyonunda; %64’ü geriatrik yaş grubunda, %80’i evli, %92’sinin düzenli geliri mevcut ve kadın erkek oranı eşit olarak tespit edilmiştir. Pandemi döneminde sigara içme parametrelerinde %15 oranında iyileşme olduğu, egzersiz davranışlarında değişme olmadığı görülmüştür. Hastaların %43'ünde sağlık hizmetlerine erişim değişmezken, %49'unun polikliniğe başvurmaktan çekindiği ve bunların %29'unun telefonla sağlık hizmeti aldığı sonucuna ulaşılmıştır. Pandemi öncesine göre ilaç kullanımı %68 hastada değişmezken, %29 hasta çok daha düzenli ilaç kullanımı bildirmiştir. Atak sıklığı, hastaların %43’ünde pandemi öncesindeki yıla göre aynı oranda, %37 hastada ise daha az atak geliştiği tespit edilmiştir. Hastaların %49’unun hastane yatışı azalmış olduğu görülmüştür. Çalışma populasyonunda ortalama CAS 1.72 olarak tespit edilmiştir. CAS ortalamalarının alt parametrelerinde anlamlı bir ilişki bulunmamıştır.
Sonuç: Hastalar, pandemi döneminde daha iyi korunma ile daha az zarar görmüşler ve güçlü anksiyete kontrolü geliştirmişlerdir. Hastaların yaklaşık yarısı pandemi sürecinde sağlık hizmeti almak için hastane başvurusundan kaçınıp, telefonla sağlık hizmeti alma alternatifini kullanmışlardır. Sağlık politikalarında detaylı olarak tasarlanacak tele-tıp uygulamaları ile hastaneye yatışlar azaltılabilir ve hastalık yönetimi kolaylaştırılabilir.
