Gebe Bir Kedide Abdominal Travmaya Bağlı Tam Uterus Rüptürü

Abstract

Sunulan çalışmada travma şikayetiyle kliniğimize getirilen Tekir ırkı gebe bir kedide uterus rüptürü olgusu ve operatif müdahale sonrası durumu anlatılmaktadır. Klinik muayenede, abdominal ağrı ve hassasiyet ile kahverengi vaginal akıntı tespit edildi. Transabdominal ultrasonografide, intraperitoneal sıvı artışı ve uterusun anterior duvarına doğru uzanan lineer anekoik saha belirlenirken fetüslerde kalp atımlarının olmadığı saptandı. Uterus rüptürü tanısı konulan gebe kedi acil operasyona alındı. Operasyon sırasında uterusun anterior duvarının rüptüre olmasıyla birlikte abdominal kavitede uterus içeriğinin ve serbest fetüslerin yer aldığı görüldü. Ölü olduğu belirlenen serbest fetüsler cıkarıldıktan sonra ovariohisterektomi ile rüptüre olan uterus ve her iki ovaryum uzaklaştırıldı. Operasyon sonrası 10 gün içinde herhangi bir komplikasyon ile karşılaşılmazken, 3 aylık takiplerde kedinin sağlıklı olduğu belirlendi. Sonuç olarak, kedilerde gebeliğin erken dönemlerinde de travmaya bağlı uterusun rüptüre olabileceği ve annenin sağlığı için erken müdahalenin önemli olduğu kanısına varılmıştır.

Complete Uterine Rupture Associated With Abdominal Trauma in A Pregnant Queen

Abstract

In this study, uterine rupture and postoperative process were described in a pregnant queen who referred to clinic due to trauma. On clinical examination, a brownish vaginal discharge, abdominal pain and tenderness were observed. Transabdominal ultrasonography revealed that increased intraperitoneal fluid and a linear anechoic area extended to anterior uterine wall were evident, whereas fetuses had no heartbeat. The surgery of pregnant queen was then immediately performed following the diagnosis of uterine rupture. During surgery, it was seen that abdominal cavity was fluid filled by uterine discharge and free fetuses existed out of uterus due to the complete rupture of uterine wall. After removal of dead fetuses from abdominal cavity, ovariohysterectomy was performed. No complication was observed within 10 days after surgery and the cat was healthy throughout 3 months scheduled visit. Therefore, it was concluded that uterine rupture might be occurred by trauma during early period of pregnancy in queens and immediate surgical intervention should be performed for maternal viability
