MALİGN MELANOM OLGULARINDA PROGNOSTİK PARAMETRELER İLE İNTRATÜMÖRAL MAST HÜCRE VARLIĞI VE EPİDERMAL DEĞİŞİKLİKLERİN KARŞILAŞTIRILMASI

Abstract

Amaç: Malign melanom(MM), insidansı giderek artan, melanositlerden köken alan bir neoplazidir. Mast hücrelerinin alerjik olaylardaki rolü iyi bilinmektedir fakat solid tümörlerde mikroçevre ve prognoz ile ilişkileri kesinlik kazanmamıştır. MM’da epidermal değişikliklerin önemli bir prognostik parametre olan ülser gelişimi ve tümör hücre büyümesi ile ilişkili olabileceği düşünülmektedir. Bu çalışmada prognostik önemi kanıtlanmış parametreler ile mast hücreleri ve epidermal değişiklikler arasındaki ilişkiyi göstermek amaçlandı.
Materyal-Metod: Çalışmamıza Ocak 2015-Şubat 2019 tarihleri arasında eksizyonel biyopsi ile MM tanısı alan 40 olgu dahil edilmiştir. Olguların H&E boyalı preparatlarından epidermal değişiklikler (bazoskuamoz dejenerasyon varlığı, incelme, rete oluşumu, epidermis-dermis arası kleft oluşumu), plazma hücre sayısı ve ek olarak yapılan Giemsa histokimyasal boyası ile intratümöral mast hücre sayısı değerlendirilmiştir. Bu bulgular prognostik parametreler ile karşılaştırılmıştır.
Bulgular: Olguların %32’si (n=13) kadın, %67.5’u (n=27) erkekti. Bazoskuamoz dejenerasyon varlığına göre clark seviyesi, ülserasyon, lenfositik infiltrasyon arasında anlamlı farklılık saptanmadı. Mast hücre sayısı ile breslow kalınlığı, ülserasyon, lenfositik infiltrasyon, mitoz sayısı ile anlamlı ilişki saptanmadı. Plazma hücre varlığına göre prognostik faktörler ile anlamlı farklılık saptanmış olup zeminde nevüs görülen olgularda plazma hücreleri sayıca daha düşüktür.
Sonuç: Olguların çoğunda epidermal değişikliklerin gözlenmesi, bu bulgunun malign melanomun histopatolojik karakteristik bulgularından biri olabileceğini düşündürmektedir. Bazı çalışmalarda epidermal değişikliklerin ülser gelişiminin öncüsü olabileceği öne sürülmektedir, bu da prognostik öneminin olabileceğini gösterir. Çalışmamızda intratümöral mast hücreleri ile prognostik parametreler arasında istatistiksel olarak anlamlı ilişki görülmedi. Mast hücrelerinin hem pro-tümor hem anti-tümör etki göstermesi ve tümörün evresine göre bu etkinin değişebileceğinin bildiren çalışmalar olması nedeniyle bu sonucun çıkmış olabileceği düşünülmektedir.
