Tuberculous Spondylitis May be The Cause of Back Pain

Abstract

Tuberculosis is the most frequent cause of spinal infections with a preferential involvement of the midthoracic and upper lumbar vertebrae. Since the onset of disease is insidious and the development of radiographic anormalities is slow; with lack of spesific symptoms, the diagnosis is delayed. Long-lasting back pain with constitutional symptoms and elevated acute phase response, tuberculous spondylitis should be considered. Magnetic resonance imaging has an important place in diagnosis. Early diagnosis and proper therapy is important in prevention of possible morbidities. We present two cases with back pain referred to our outpatient department, not responding to medical therapy and aim to discuss diagnostic and therapetic aspects.

Sırt Ağrısı Nedeni Tüberküloz Spondilit Olabilir

Abstract

Spinal enfeksiyonların en sık nedeni tüberküloz olup, tercihen orta torakal ve üst lomber vertebralar tutulur. Hastalığın başlangıcının sinsi, radyografik bulgularının yavaş seyirli olması ve spesifik semptomlarının olmamasıyla tanı gecikir. Uzun süren bel ve sırt ağrısının yanı sıra, konstitüsyonel semptomlar ve akut faz cevabının yüksek olması tüberküloz spondiliti düşündürmelidir. Manyetik rezonans görüntüleme tanıda önemli yer tutar. Erken tanı ve uygun tedavi hastanın gelişebilecek morbiditesinin önlenmesinde büyük önem taşır. Polikliniğimize medikal tedaviye dirençli sırt ve bel ağrısı ile başvuran iki tüberküloz spondilit olgumuzu tanı ve tedavi yaklaşımları ile tartışmak üzere sunuyoruz.
