“Evaluation of Patients with Globus Hystericus with Polisomnography”

Abstract

Aim: Aim of this study is to investigate the sleep architecture and the role of obstructive sleep apnea OSA in the etiology and differential diagnosis of globus hystericus patients with polisomnography. Material and method: Nine adult patients > 18 years with the diagnosis of globus hystericus and without any known complaints related to underlying diseases were included to the study. All the patients were examined with detailed history taking. Spirometric lung function tests were made and were evaluated with flow volume curves to exclude upper airways obstruction. They were asked about sleep diturbances before polysomnography. Control group consisted of the cases who had apnea–hypopnea index of < 5 in our sleep research laboratory. Thirty-five females without any psychiatric diseases met these criteria. Results: All the patients were female with a mean age of 47±9 years range 35-62 . Chest X-rays were normal. Flow–volume curves did not show any pattern as saw tooth patern of OSA patients related to upper airways diseases. ENT examination did not reveal any pathology leading to the feeling of globus hystericus in the oropharyngeal region. Mean neck circumference was 34.6±2.3 cms range 32-38 and mean BMI was 27.31±6.33 kg /m2 range 21.70-33.90 . Mean duration of the symptoms of globus hystericus was 5.6±9.94 years range 36 days- 30 years . Epworth Sleepiness Score was 4±4 range 0-10 and they did not experience excessive day time sleepiness. In 3 of 9 patients we diagnosed mild and moderate OSA 33.3% of all the cases . When sleep patern of those patients were examined we found that duration of the sleep latency was longer than the control group statistically significant, p=0.027 . Other polysomnography parameters were within the limits of normals. Conclusion: This study is the first in the literature concerning sleep architecture of globus hystericus patients by the use of polysomnography. In 3 of 9 patients we diagnosed OSA 33.3% of all the cases and we found that the duration of sleep latency of those patients was longer than the control group.

“Globus Histerikus Tanılı Hastaların Polisomnografi ile Değerlendirilmesi”

Abstract

Amaç: Bu araştırma; globus histerikus tanılı hastaların polisomnografi ile uyku yapılarını incelemek ve hastalığın ayırıcı tanısında yada etiyolojiside obstrüktif sleep apne OSA varlığını araştırmak için tasarlanmıştır. Yöntem: Çalışmaya “Globus Histerikus” tanılı ve globus yakınmasına yol açabilecek altta yatan bir hastalığı olmadığı bilinen 9 erişkin ≥18 yaş hasta alınmıştır. Tüm hastalar detaylı sorgulanmış, muayene edilmiş ve solunum fonksiyon testi ile spirometrik inceleme yapılarak üst solunum yolu obstrüktif patolojileri açısından akım-volüm halkası ile değerlendirilmiştir. Polisomnografi öncesi, uykuyla ilgili yakınmaları sorgulanmıştır. Çalışma süresince Göğüs Hastalıkları Kliniği Uyku Bozuklukları Laboratuvarı’nda polisomnografi ile tetkik edilen ve apnehipopne indeksi
