POSTSPİNAL BİLATERAL İNTRAKRANYAL SUBDURAL HEMATOM (OLGU SUNUMU)

Abstract

Giriş ve Amaç: Spinal anestezi sonrası gelişen intrakranyal subdural hematom, nadir görülen ve tedavi edilmediğinde öldürücü olabilen ciddi bir komplikasyondur. Ponksiyon sırasındaki dural zedelenme nedeniyle devam eden beyin omurilik sıvısı (BOS) sızıntısı, meningiyal damarlarda gerilme ve yırtılma ile kanamaya yol açmaktadır. Yazımızda genel anestezi altında “burr-hole” ile dekompresyon operasyonuna aldığımız, 10 gün önce spinal anestezi denenen, başarılı olmayınca genel anestezi altında sezaryen operasyonu geçiren, hastanemize başvurduğunda postspinal bilateral intrakranyal subdural hematom teşhisi konan olguyu literatür bilgileri ışığında tartışmayı amaçladık. Olgu: On gün önce sezaryen operasyonu geçiren 35 yaşındaki kadın olgunun operasyon sonrası konservatif tedavilere yanıt vermeyen baş ağrısı şikayeti olmuş. Şiddetli baş ağrısı ile acil servise başvuran hastaya subdural hematom tanısı konmuş ve konvülziyon geçirmesi üzerine acil şartlarda genel anestezi altında dekompresyon operasyonuna alındı. Tartışma ve Sonuç: Başarısız bile olsa spinal anestezi denemelerinden sonra, sebat eden baş ağrısı varlığında subdural hematom olabileceği düşünülerek, dikkatli öykü alınmalı, radyolojik görüntüleme tetkikleri ile erkenden tanı konmalıdır. Baş ağrısını gidermede verilen analjeziklerin tabloyu maskeleyebileceği hatırda tutulmalıdır.
