Aim: We present a patient who had the right coronary artery arising from left sinus of valsalva which is a very rare congenital anomaly and may cause myocardial infarction and sudden death. The relevant literature is also reviewed. Case: A 55 year-old woman was hospitalized due to acute coronary syndrome. Coronary angiographic examination revealed a right coronary artery arising from left sinus of valsalva which was totally occluded at the proximal part of vessel. Conclusion: Anomalous origin of right coronary artery from the left sinus of valsalva is a rare and malignant anomaly which can lead to myocardial infarction.
