Sunumu Ovarian Strumal Karsinoid: Olgu

Abstract

Strumal karsinoid çoğu trabeküler tipte karsinoid tümör ile tiroid elementlerininin
karışımından oluşan over teratomlarının nadir görülen bir formudur. Ovarian
kitlenin büyümesine bağlı semptomlarla ya da insidental olarak ortaya çıkar.
Kasık ve bel ağrısı olan elli yaşında kadın hasta hastanemiz kadın hastalıkları ve
doğum polikliniğine başvurdu. Sistem muayenesi normal olan hastaya yapılan alt
abdominal bilgisayarlı tomografide sol adneksiyal alanda 16x12 cm boyutta solidgeniş
kistik kitle lezyonu saptanarak öncelikle over tümörü lehine olduğu belirtildi.
Adneksiyal kitle ve myoma uteri nedeniyle total abdominal histerektomi+bilateral
salpingooferektomi yapılan hastada overde strumal karsinoid ve çevresinde
yağ doku alanları ile iç içe deri eklerinin varlığı saptandı. İmmünohistokimyasal
incelemede tiroid dokusu alanlarında tiroid transkripsiyon faktör-1 ve tiroglobulin
pozitifliği, karsinoid alanında ise sinaptofizin ve kromogranin pozitifliği izlendi.
Over teratomlarının nadir görülen bir varyantı olan strumal karsinoid tanılı
hastanın klinik, morfolojik ve immünohistokimyasal özellikleri literatürler eşliğinde
tartışılarak sunuldu.

Strumal Carcinoid of the Ovary: A Case Report

Abstract

Strumal carcinoid is a rarely seen form of ovarian teratoma composed of an mixture of mostly trabecular carcinoid tumor and thyroid elements. It is detected based on the symptoms resulting from the growth of an ovarian mass or incidentally. A 50-year-old female patient with inguinal and low back pain was admitted to the outpatient clinic of the gynecology and obstetrics department in our hospital. The patient’s systemic examination did not reveal any abnormalities but her lower abdominal computed tomography which in turn showed a solid and large cystic mass with dimensions of 16x12 cm in the left adnexal region. First, these findings were considered in favor of an ovarian tumor. The patient underwent total abdominal hysterectomy+bilateral salpingo-oopherectomy operation for the adnexal mass and uterine myomas. We detected stromal carcinoid in the ovary and skin appendages nested with adipose tissue areas in the surrounding area. In the immunohistochemical examination, thyroid transcription factor-1 and thyroglobulin positivity was observed in the areas of the thyroid tissue and, synaptophysin and chromogranin positivity was observed in the carcinoid area. We present clinical, morphological and immunohistochemical characteristics of a patient diagnosed with stromal carcinoid, a rarely seen variant of the ovarian teratomas, along with the literature.
