Penis Kırığı Tanı ve Tedavisinin Klinik Sonuçları: Son 10 Yıldaki Deneyimimiz

Abstract

Amaç: Çalışmamızın amacı, son on yıl içindeki kapsamlı klinik deneyime dayanarak, penil fraktürlerin klinik özelliklerini, tanı yöntemlerini ve cerrahi sonuçlarını değerlendirmektir. Penil fraktür (PF) iyi bilinen bir klinik durum olmasına rağmen, bu çalışmanın özgünlüğü, hem tanı yaklaşımlarının hem de cerrahi sonuçların detaylı bir şekilde analiz edilmesindedir. Bu yönleri inceleyerek, çalışmamız sadece bu nadir ürolojik yaralanmanın yönetimine dair değerli bilgiler sağlamakla kalmayıp, aynı zamanda post-operatif komplikasyonlar hakkında detaylı bilgiler sunarak, PF tedavisi hakkında uzun dönemli bir klinik perspektiften daha derin bir anlayışa katkıda bulunmaktadır.
Yöntemler: Mart 2014 ile Mart 2024 tarihleri arasında penil fraktür nedeniyle ameliyat edilen 33 hasta üzerinde geriye dönük olarak tıbbi kayıtlar sistematik olarak incelendi. Epidemiyolojik veriler, hasta öyküsü, klinik sunum, etiyoloji, ameliyat bulguları ve ameliyat sonrası komplikasyonları analiz edildi. Cerrahi müdahale sirkumferansiyel subkoronal insizyon ile penis derisinin soyulması ve tunika yırtığının onarımını içeriyordu. İstatistiksel analiz IBM SPSS Statistics kullanılarak yapıldı.
Bulgular: Hastaların ortalama yaşı 41.9 ± 13.17 yıldı. Yaralanmadan acil servise başvuruya kadar geçen süre ortanca 5 saatti (1-24 saat aralığında). En yaygın etiyoloji %57,6 ile cinsel ilişki sırasında gelişen penis travmasıydı. Tüm hastaların tıbbi öykülerinde ve fizik muayenelerinde hematom saptandı. Olguların %36,4'üne penis dopler ultrasonografisi uygulandı ve bu olguların tümünde kavernozal rüptür tespit edildi. Tüm hastalara yaralanmadan sonraki 24 saat içinde cerrahi onarım uygulandı. Ortalama hastanede kalış süresi 1 gündü. Erken komplikasyon görülmedi ve takip sırasında hiçbir hastada erektil disfonksiyon ve penil kurvatür gelişmedi.
Sonuç: Penil fraktür tanısında anamnez ve fizik muayene genellikle yeterlidir. Tanıda belirsizlik durumunda, deneyimli radyologlar tarafından yapılan penil ultrasonografi etkili bir tanı aracı olabilir. Hastaneye başvuruya kadar geçen süre de dahil olmak üzere, hastaların yaralanmadan itibaren 24 saat içinde cerrahi müdahale edilmesi, erektil disfonksiyon ve penil eğrilik riskini önemli ölçüde azaltarak, bu hastalarda daha iyi fonksiyonel sonuçlar elde edilmesini sağlar.

Clinical Results of Diagnosis and Treatment of Penile Fracture: Our Experience of the Past 10 Years

Abstract

Purpose: This study aims to evaluate the clinical characteristics, diagnostic methods, and surgical outcomes of penile fractures based on the last 10 years of experience while providing insights into managing this rare urological injury.
Methods: A retrospective review was conducted on 33 patients who underwent surgery for penile fractures between March 2014 and March 2024. Medical records were systematically reviewed to obtain epidemiological data, patient history, clinical presentation, etiology, operative findings, and postoperative complications. The surgical intervention included a subcoronal degloving incision and repair of the tunica albuginea. Statistical analysis was performed using IBM SPSS Statistics.
Results: The mean age of patients was 41.9 ± 13.17 years. The median time from the injury to presentation at the emergency department was 5 hours (with a range of 1 to 24 hours). The most common etiology was trauma during sexual intercourse 57.6%. All patients exhibited hematomas in their medical history and physical examination. Penile ultrasound was performed in 36.4% of cases, detecting cavernosal rupture in all cases. Surgical repair was performed within 24 hours of injury for all patients, with a median hospitalization of 1 day. No early complications occurred, and none of the patients developed erectile dysfunction and penile curvature during follow-up.
Conclusion: Early surgical intervention is crucial for optimal outcomes in penile fracture cases. Penile USG can be an effective diagnostic tool, particularly in experienced centers. Prompt admission, diagnosis, and repair significantly reduce the risk of complications such as erectile dysfunction and penile curvature.
