Genel anestezi altında gerçekleştirilen sezaryen ameliyatlarında traneksamik asidin hemoglobin değişimleri üzerindeki doz bağımlı etkileri: retrospektif bir analiz

Abstract

Amaç: Bu çalışma ile genel anestezi altında sezaryen ameliyatı geçiren hastaların tıbbi kayıtlarını retrospektif inceleyerek, traneksamik asidin (TXA) farklı dozlarda perioperatif kan kaybını azaltmadaki etkinliğini, hemoglobin ve hematokritte oluşan değişiklikler üzerinden değerlendirmeyi amaçladık. Yöntem: Çalışma, üçüncü basamak bir üniversite hastanesinde gerçekleştirilmiştir. Ocak 2019 ile Eylül 2020 tarihleri arasında genel anestezi altında sezaryen ameliyatı geçiren hastaların tıbbi kayıtları incelenmiştir. Hastalar üç gruba ayrılmıştır: Grup 1 (TXA uygulanmayan), Grup 2 (500 mg TXA) ve Grup 3 (1000 mg TXA). TXA uygulaması dışında, tüm gruplarda klinik ve demografik veriler, cerrahi endikasyonlar, ameliyat süresi, hemoglobin değişimleri ve kan ürünü transfüzyonları analiz edilmiştir. Bulgular: Toplam 150 hasta çalışmaya dahil edilmiştir. Grup 3'teki hastaların, diğer gruplara kıyasla ameliyat öncesi hemoglobin (Hb) ve hematokrit (Hct) düzeyleri ile ameliyat sonrası Hb, Hct değerleri anlamlı olarak daha yüksekti. Ameliyat öncesi ve sonrası Hb düzeylerindeki fark, Grup 3’te daha düşüktü ve bu bulgu istatistiksel olarak anlamlıydı (p=0.045). TXA verilen gruplarda kreatinin düzeyleri kontrol grubuna kıyasla anlamlı olarak daha düşüktü. Ayrıca Grup 3’te yenidoğanların 1. ve 5. dakikalık APGAR skorları anlamlı olarak daha yüksekti. Grup 3 ayrıca daha az postpartum uterotonik ajana (oksitosin vb.) ihtiyaç duymuş; hastanede ve yoğun bakım ünitesinde (YBÜ) kalış süreleri daha kısa bulunmuştur. Sonuç: Bu çalışma, genel anestezi altında yapılan sezaryen ameliyatları sırasında perioperatif hemoglobin düşüşünün en aza indirilmesinde 1000 mg TXA uygulamasının etkili olabileceğini göstermektedir. Ancak, optimal doz stratejisini doğrulamak ve bu hasta popülasyonunda TXA'nın uzun vadeli güvenliğini ve etkinliğini daha iyi anlamak için geniş ölçekli, prospektif çalışmalara ihtiyaç vardır.

Dose-dependent effects of tranexamic acid on hemoglobin changes in cesarean sections performed under general anesthesia: a retrospective analysis

Abstract

Aim: This study aimed to evaluate the efficacy of different doses of tranexamic acid (TXA) in reducing perioperative blood loss by retrospectively analyzing the medical records of patients who underwent cesarean section under general anesthesia, focusing on changes in hemoglobin and hematocrit levels. Methods: This study was conducted in a tertiary university hospital. The medical records of patients who underwent cesarean section under general anesthesia between January 2019 and September 2020 were reviewed. The patients were divided into three groups: Group 1 (control group/no TXA), Group 2 (500 mg TXA), and Group 3 (1000 mg TXA). In addition to TXA administration, clinical and demographic data, surgical indications, operation duration, hemoglobin changes, and blood product transfusions were analyzed across all groups. Results: A total of 150 patients were included in the study. Patients in Group 3 had significantly higher preoperative hemoglobin (Hb) and hematocrit (Hct) levels, as well as higher postoperative Hb and Hct values, compared to the other groups. The decline between preoperative and postoperative Hb levels was lower in Group 3, and this finding was statistically significant (p=0.045). Creatinine levels in the TXA-administered groups were significantly lower compared to the control group. Additionally, the 1- and 5-minute APGAR scores of the newborns were significantly higher in Group 3. Furthermore, Group 3 required fewer postpartum uterotonic agents (e.g., oxytocin) and had shorter lengths of stay in both the hospital and the intensive care unit (ICU). Conclusion: This study indicates that administering 1000 mg of TXA may be effective in minimizing perioperative hemoglobin decline during cesarean sections performed under general anesthesia. However, large-scale, prospective studies are needed to confirm the optimal dosing strategy and to better understand the long-term safety and efficacy of TXA in this patient population.
