Abstract Comparative Investigation about Influence of Diltiazem and Lidocaine on Haemodynamic Response to Extubation in the Patients with Hypertension Aim: The hemodynamic response during extubation may be a serious cause of morbidity and mortality in hypertensive patients in particular. The supression of hemodynamic response is essential for postoperative prognosis. It was aimed to investigate the possible hemodynamic changes after iv bolus administration of diltiazem and lidocaine following the cessation of anesthetic agents in hypertensive patients undergoing elective total hip and/or knee artroplasty. Method: After obtaining the approval of the local Ethical Committee and patients\' consent, 60 hypertensive patients undergoing elective total hip artroplasty and total knee artroplasty in the age range of 18-60 in ASA II-III group were randomly divided into 3 groups. Following the standard general anesthesia induction, intraoperative hemodynamic changes were recorded. Two minutes prior to the extubation, 5 mL saline was administered by i.v. route to the control group (K) whereas 1 mg/kg lidocaine (L) was injected intravenously to Lidocaine (L) group and and 0.2 mg/kg diltiazem to diltiazem group (D). During extubation and after extubation untill the 10th minute of application, the quality of extubation were assesed on the basis of a five point scale. The heart rates, sistolic and diastolic arterial pressure values, mean arterial pressure, and pulse pressure product values were determined at certain time intervals (1., 2., 3., 5., and 10. mins prior to the induction, before and following the drug administration, after extubation). Results: The demographic data of all groups were similar. However, the percentage changes in sistolic arterial pressure values from baseline to the postextubation at the 3rd, 5th and 10th minutes were significantly different in favour of group D compared to group L. Similarly, the percentage changes in diastolic arterial pressure values from baseline to the postextubation at the 1st, 3rd, 5th and 10th minutes were significantly different in favour of group D compared to group L. Regarding the quality of extubation, all groups were similar. Conclusion: It can be suggested that the intravenous bolus administration of diltiazem at the dose of 0.2 mg/kg might be much efficient than lidocaine i.v. bolus administration at the dose of 1 mg/kg in order to control hemodynamic response during extubation.
Amaç: Ekstübasyona bağlı gelişen hemodinamik yanıt özellikle hipertansif hasta grubunda ciddi morbidite ve mortalite nedenidir. Hemodinamik yanıtın baskılanması bu olguların postoperatif prognozu açısından önemlidir. Çalışmada elektif total kalça artroplastisi ve total diz artroplastisi uygulanan hipertansif hastalarda anestezik ajanların kesilmesiyle ortaya çıkabilecek olan hemodinamik değişiklikler üzerine intravenöz bolus olarak kullanılan diltiazem ve lidokainin etkilerinin araştırılması amaçlandı. Yöntem: Etik kurul izni ve hasta onayı alınarak elektif total kalça artroplastisi ve total diz artroplastisi uygulanan, ASA 2-3 grubu, 18-60 yaş arası, hipertansif 60 hasta rastgele üç gruba ayrıldı. Standart genel anestezi indüksiyonu sonrasında intraoperatif hemodinamik değişimler kaydedildi. Ekstübasyondan 2 dakika önce kontrol (K) grubuna 5ml salin iv, lidokain (L) grubuna 1 mg/kg lidokain iv ve diltiazem (D) grubuna 0.2 mg/kg diltiazem iv bolus olarak verildi. İndüksiyon öncesi, ilaç öncesi, ilaç sonrası, ekstübasyon, ekstübasyon sonrası 1.,2.,3.,5. ve 10. dakikalardaki kalp atım hızı, sistolik arter basıncı, diyastolik arter basıncı, ortalama arter basıncı, nabız basınç ürünü değerleri kaydedildi. Ekstübasyonda ve ekstübasyon sonrası 10. dakikaya kadar olan dönemde trakeal ekstübasyon kalitesi 5 puanlı skala ile değerlendirildi. Bulgular: Tüm grupların demografik verileri benzerdi. Başlangıca göre sistolik arter basıncındaki yüzdesel değişimlerin gruplara göre değerlendirmesinde ekstübasyon sonrası 3, 5, 10. dakikalardaki grup L ve grup D arasındaki fark grup D lehine anlamlıydı. Benzer olarak başlangıca göre diastolik arter basıncındaki yüzdesel değişimlerin gruplara göre değerlendirmesinde ekstübasyon sonrası 1, 3, 5 ve 10. dakikalarda grup L ve grup D arasındaki fark grup D lehine anlamlıydı. Ekstübasyon kalitesi açısından ise tüm gruplar benzerdi. Sonuç: Ekstübasyon esnasında hemodinamik yanıtı kontrol etmede 0.2 mg/kg diltiazem iv bolus 1mg/kg lidokain iv bolusa göre daha etkin olabilir.

In March 2015, Prof. Dr. Caferi Tayyar Şaşmaz undertook the Editor-in Chief position and since then he has been in charge.
Publishing in three issues per year (April - August - December), it is a multisectoral refereed scientific journal. In addition to research articles, scientific articles such as reviews, case reports and letters to the editor are published in the journal. Our journal, which has been published via e-mail since its inception, has been published both online and in print. Following the Participation Agreement signed with TÜBİTAK-ULAKBİM Dergi Park in April 2015, it has started to accept and evaluate online publications.
