Strategies for increasing treatment adherence in bipolar disorder

Abstract

As soon as a patient is diagnosed with bipolar disorder, his/her life changes permanently. The probability of swinging from one pole to another effects his/her life in almost all aspects; in addition to that, he/she is also introduced with medications that adds substantial burden to their lives. They are expected to take medications that might cause serious side effects that alters their life, body image and self esteem such as weight gain, hair loss, excessive sleep etc., for an unknown period of time. Additionally, none of those medicines provide any hope for cure but an improvement in symptoms and a delay for the episodes. All these features increases the possibility of nonadherence to follow up visits and treatments in bipolar disorder patients. Establishing a strong alliance with patients and caregivers can help physicians monitor for adherence problems and introduce interventions as necessary. Physicians should provide education to help patients accept their diagnosis and the severity and chronicity of their illness. Simplifying patients’ medication regimens and monitoring for and treating side effects can help patients maintain treatment adherence and achieve optimal outcomes.

İki uçlu bozuklukta tedavi uyumunu arttırma stratejileri

Öz

Hastalar iki uçlu bozukluk tanısı aldıkları gün hayatları değişir. Bir taraftan iki uçlu hastalığın kutupları arasında hayatının tüm yönlerini etkileyen bir hastalıkla, diğer taraftan bu durumu tedavi etmek amacıyla verilen ilaçların hayatlarına getirdiği yükle uğraşmak zorunda kalırlar. Kendilerinden, yaşamlarını, benlik saygılarını ve vücut algılarını etkleyebilen kilo alma, saç dökülmesi, aşırı uyku gibi ciddi yan etkiler yapan ilaçları bilinmeyen bir süreyle almaları beklenmektedir. Öte yandan, bu ilaçlar iyileşme değil, yalnızca belirti kontrolü ve atak aralıklarını açma vaad etmektedir. Tüm bu nedenler iki uçlu hastalarda tedaviye uyumun bozulmasına yol açabilir. Tedaviye uyum problemlerinin saptanması ve en kısa sürede müdahalesi için hasta ve hasta yakınlarıyla güçlü bir işbirliğinin kurulması son derece gereklidir. Psikiyatrist hastasını hastalığın şiddeti ve kronisitesi ile ilgili bilgilendirmeli ve tanısını kabul etmesini sağlamalıdır. Hastanın tedavi rejiminin basitleştirilmesi ve yan etkilerin saptanarak müdahale edilmesi tedaviye uyumu ve optimal tedavi sonucuna ulaşma olasılığını arttırır.
