Abstract

tedavisinde kullanımı çok uzun yıllara dayansa da bu konudaki araştırmalar oldukça azdır. 1990’ların başına kadar yapılmış bazı açık etiketli çalışmalar ve olgu serileri faydalı olabileceklerini bil- dirmiştir. Daha sonraki yıllarda metilfenidat ile ilgili yapılan hem prospektif hem de geriye dönük desende yapılan çalışmalar da açık etiketli olmuştur. Güncel literatür bu ajanların iki uçlu dep- resyonda kullanımı ile ilgili randomize kontrollü çalışmalardan hala yoksundur (40). Mizaç dengeleyicilere yetersiz yanıtı olan 85 iki uçlu depresyon hastası ile yapılmış plasebo kontrollü ekleme tedavisini test eden bir çalışma modafinil kullanan hastalarda daha yüksek antidepresif yanıt bildirmiştir. Tedaviye ikincil hipomani veya mani açısından modafinil plaseboya eşdeğer bul- muştur (41). Bu alanda yayınlanmış en yeni randomize kontrollü çalışma lityum, olanzapin veya valproik asit kullanmakta olan 258 iki uçlu I depresyon hastasında plaseboya karşı armodafinil eklemenin depresif belirtileri iyileştirmede daha üstün olduğunu göstermiştir (42). Tüm çalışmalar birlikte ele alındığında iki uçlu depresyon tedavisinde ekleme stimülan kullanımının yorgunluk, uykuya meyil ve bilişsel problemlere genel olarak faydalı olduğu, etkilerinin 1-2 hafta gibi kısa bir sürede başladığı ve erken dönemde yanıtın varlığının devam edecek iyileşmenin de haber
