ANKARA’DA DİŞ TEKNİSYENİ PNOMOKONYOZU SIKLIĞI VE İLİŞKİLİ OLDUĞU FAKTÖRLER

Abstract

Giriş: Diş teknisyenlerinde tozun neden olduğu parankimal değişiklikler pulmoner fonksiyonları etkilemektedir. Bu çalışmada, Ankara’da görev yapan diş teknisyenlerinde pulmoner tutulumun derecesini gösteren radyolojik skorlar Akciğer grafiği / YRBT ile fonksiyonel bulguların etkilenme derecesi arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Gereç ve Yöntem: Ankara ilinde görev yapan 440 diş, teknisyeni akciğer grafisi, solunum fonksiyon testleri, yüksek çözünürlüklü bilgisayarlı tomografi ile değerlendirildi. Radyolojik görüntülerdeki pulmoner tutulum derecesi; opasitelerin yoğunluğu, amfizem, buzlu cam, bal peteği, plevral anormalliklerin genişliği temel alınarak kategorize edildi. Bulgular: Çalışmaya katılanların ortalama yaşı 34,7±8,5 idi ve ortalama çalışma süresi 16,5±7,9 yıl idi. Diş teknisyeni pnömokonyozu DTP prevalansı %11,1 idi. YRBT’de en yaygın opasite %89,7 ile round opasite idi. Büyük opasite oranı ise %14,3 idi. ILO International Labor Organization skoru ve YRBT skoru arasında pozitif korelasyon vardı. ILO ve YRBT skorları ile tüm solunum foksiyon testleri FEV1/FVC dışında arasında negatif korelasyon saptanırken, yaş total maruziyet süresi, sigara, işe başlama yaşı arasında bir ilişki saptanmadı. İleri istatistiksel analizde; pulmoner tutulumun derecesi YRBT skoru fonksiyonel bozukluğun bağımsız belirleyicisi olmuştur. Pulmoner tutulumun derecesini en iyi yansıtan fonksiyonel parametre ise %FEV1 idi.Sonuç: Biz diş teknisyenlerinde mix toz maruziyeti ile ilişkili fonsiyonel bozuklukların gösterilmesinde radyolojik ve fonksiyonel ölçümlerin birlikte kullanılması gerektiğini düşünüyoruz.
