Abstract

Amaç: Over yüzey epiteli ve inklüzyonlarından köken aldığı düşünülen müsinöz over tümörü patogenezinde tuba epiteli kökenini savunan tubadan dökülme teorisini araştırmak. Yöntem: Hastanemizde 2008-1012 tarihleri arasında tanı alan tüm over tümörleri tarandı. Patoloji camları benign ve borderline gruplarda over yüzey epiteli, over inklüzyonları ve aynı taraf tuba epiteli değişiklikleri (nükleer pleomorfizm, nükleer stratifikasyonun varlığı, hücrelerin papillomatozis oluşturması) değerlendirildi. Bulgular: Kistadenom vakalarında over yüzey epitelinde papillomatozis ve nükleer stratifikasyon yoktu. Nükleer pleomorfizm%71; yarıklanma%21 ve inklüzyon kisti %42,8 oranında görülmüştür. İnklüzyon epitelinde papillomatozis izlenmiyorken nükleer stratifikasyon %33, nükleer pleomorfizm %100 oranında saptanmıştır. Bu vakaların tuba epitelindeki değişiklikleriyse stratifikasyon/pleomorfizm %100. Borderline vakalarında over yüzey epitelinde papillomatozis ve nükleer stratifikasyon yoktu. Nükleer pleomorfizm %77,7; yarıklanma %44; inklüzyon kisti %77,7 oranında görülmüştür. İnklüzyon epitelinde papillomatozis %14; nükleer stratifikasyon %57 ve nükleer pleomorfizm %100 oranında saptanmıştır. Bu vakaların tuba epiteli değişiklikleri şöyleydi: Papillomatozis %75; nükleer stratifikasyon/pleomorfizm %100. Sonuç: Tubadan dökülme teorisi, müsinöz tümör gelişimini açıklamamaktadır
