Pleomorphic adenoma of the lacrimal gland: A case report

Abstract

Pleomorphic adenoma (PA) is a benign tumor that most commonly arises from the major salivary glands; involvement of the lacrimal gland is rare. Lacrimal gland pleomorphic adenomas (LGPAs) typically present in middle-aged patients as a slowly progressive, painless, unilateral proptosis. Although histologically benign, incomplete surgical excision or biopsy procedures that violate the tumor capsule may lead to recurrence and increase the risk of malignant transformation. In this report, we present a rare case of LGPA in a 47-year-old man who presented with redness, itching, and blurred vision in the left eye. Orbital magnetic resonance imaging revealed a well-circumscribed, contrast-enhancing mass arising from the lacrimal gland, measuring 24 × 19 mm. The lesion was completely excised via a frontotemporal craniotomy, with preservation of the capsule. Postoperative imaging showed no residual tumor, and no recurrence or deformity was observed during 1.5 years of follow-up. Successful treatment of LGPA depends on accurate preoperative diagnosis and complete surgical excision with preservation of the tumor capsule. When radiological and clinical findings are consistent with LGPA, biopsy is generally discouraged, as disruption of the capsule may increase the risk of recurrence and malignant transformation. Therefore, early diagnosis and an appropriate surgical approach are crucial in the management of LGPA
