Cocoa and theobromine in neurodegeneration: Neuroprotective and cannabinoidergic mechanisms

Abstract

This review aims to critically evaluate current experimental and clinical evidence on the neuropsychopharmacological effects of theobromine and endocannabinoid-related compounds, including anandamide and N-acylethanolamines (NAEs), in cocoa and chocolate, and their relevance to neurodegenerative diseases. Endocannabinoid system–related compounds are present in cocoa and chocolate; however, the amounts required to induce direct psychoactive effects cannot be achieved through dietary consumption. NAEs may indirectly modulate endocannabinoid signaling via inhibition of fatty acid amide hydrolase (FAAH), leading to increased endogenous anandamide levels and potentially contributing to the hedonic and neuropsychopharmacological effects of chocolate. In parallel, theobromine shows favorable pharmacokinetic properties, while preclinical studies suggest potential antioxidant, anti-inflammatory, and neuroprotective effects of theobromine and other cocoa-derived compounds in models relevant to Alzheimer’s and Parkinson’s diseases. Although experimental findings support modulation of oxidative stress, neuroinflammation, and synaptic dysfunction, clinical evidence remains limited and heterogeneous. Additionally, cocoa processing affects polyphenol content and bioactivity, and combinations with other neuroprotective nutraceuticals may provide synergistic benefits. Overall, cocoa-derived compounds particularly theobromine represent promising modulators of neurodegeneration-related pathways; however, standardized, well-designed, and long-term clinical studies are needed to define their therapeutic relevance, optimal dosing, and safety.
